How can I manage quick ejaculation due to sensitive penis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am experiencing two concerns related to my sexual health. Firstly, I notice that pre-ejaculate fluid is released from my penis within one to two minutes whenever I talk to my partner, hold hands with her, or even think about her. This response occurs even when the conversation is not explicitly sexual. Secondly, I find that the sensitivity of my penis is extremely high. For example, after two to three minutes of external stimulation, such as a handjob over clothing, I ejaculate. Since I have never had sexual intercourse, I am uncertain about how long I would be able to last during actual intercourse.

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

What you are experiencing is more common than you might think, and it can certainly be improved with the right approach. You have mentioned two primary issues: the release of pre-ejaculate fluid very quickly, even with mild emotional or physical arousal, and very high penile sensitivity that leads to rapid ejaculation with minimal stimulation. These concerns are likely related to a heightened psychological and physical responsiveness, which may be due to new sexual experiences after a long period without intimacy. The high sensitivity of the penis could be physiological, such as due to a thin foreskin, an exposed glans, or nerve hypersensitivity, or it could be influenced by anxiety. Performance anxiety or focusing too much on arousal may also increase physical responses, such as the early release of precum and quick ejaculation.

For managing excess precum and a heightened arousal response, it is important to know that while this is not harmful, it does indicate a low arousal threshold. Practicing mindfulness or using distraction techniques during intimate moments can help. Gradually increasing emotional and physical closeness without involving sexual stimulation can help train both the mind and body to remain calm.

To address high sensitivity and early ejaculation, behavioral techniques such as the start-stop technique, where stimulation is paused just before climax, and the squeeze technique, gently squeezing the penis below the glans when nearing climax, can be effective. Pelvic floor exercises (kegels) are also beneficial; these involve contracting the muscles used to stop urine flow, holding for five seconds, and repeating this ten times, three times daily. Using a thicker condom during hand stimulation, even without penetration, may help reduce sensation and extend the duration before ejaculation.

If these strategies do not lead to improvement, it is important to follow up. The probable diagnosis is premature ejaculation, and the recommended treatment plan involves behavioral therapy and counseling.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
