Hello doctor,
I have a recurring red patch on my penis foreskin. It inflames after sex or masturbation. I have been using Triamcinolone for a while. After a few days, it goes away. But always returns. This patch appears on my penis foreskin and is always there and it increases after sex and masturbation. It feels sensitive during that time. It is been there for at least three years.

I have gone through the history and the picture attached (the attachment was removed to protect the patient's identity). There appears to be a red plaque over the penis. The possible differentials include Zoon's balanitis and psoriasis. Zoon's balanitis is believed to happen due to friction. Triamcinolone (Cetyl Alcohol and Glycerin) is a potent steroid cream and there is a higher chance of dependence. Also, repeated usage can thin out the skin. It would be better if you could shift to 0.1% Tacrolimus (Mineral oil and Paraffin) ointment for local application once daily at night instead of steroids. Recurrences are expected to happen. Use adequate lubrication during sex or masturbation, and wear loose-fitting innerwear.

In some developed countries this is not an OTC (over-the-counter) product and you would require a prescription from your local doctor as online prescription would not be valid. Some developing countries do not need a prescription and you can get it easily in medical store.
