Thanks for the reply.

So, can you see the darker bits of poop that have some ridges in them? I sometimes have that. It is more noticeable because it is darker and harder and mixed with lighter and softer (still brown) poop. And let me put you into context. For example, three months ago, my gut did not like some of the probiotics I took.

I started having loads of issues like increased burping (I do not burp at night or lying down in bed), and my stools are now formed but not super hard. They are pretty much between types 4 and 5 most of the time. I tried to increase my fiber by adding flax to my meals, but that does not seem to make my stools more solid.

I did see a GI (Gastrointestinal) specialist who thought my problem was mostly anxiety and told me to give it a couple more months because things were getting better. I believe I have some sort of dysbiosis. But in all honesty, I did not check it as I do now, so I have no idea. When I poop, it does not feel any different.

I have reviewed the attached pictures (attachments removed to protect the patient's identity). This is a normal spectrum of stools; do not worry about the ridges that you have mentioned. Regarding stool color, fresh blood, maroon stools like clotted blood, and dark black stools are the three types that are red flag signs.

The rest of the variations are considered normal. Another important point to note is that gut movements and the number and consistency of stools are maintained by the autonomic nervous system, so it is not in our control. Do not focus too much on this, as it will induce health anxiety and lead to abnormal frequency and consistency of stools.

I agree with the possibility of dysbiosis, and I feel that daily yogurt intake, which is a natural source of probiotics, would help you out. There is a wide spectrum of commercially available probiotics, and you can try any other one. Most of the time, probiotics are safe and are one of the best ways to maintain normal gut health.

If those ridges seem like a separate piece of poop, is it fine? I believe it is older poop mixed with new one, but I want to be sure. Probiotic pills were what started all this, so I do not want to try again. I was seriously very unwell for two weeks after. So can those sorts of stools be considered normal even if they are soft?

I have one more question, and I suspect this is my Health Anxiety speaking, so I thought I would ask.So yesterday I had a BM, and after, as usual, dissecting my poop in the middle of it saw 2 tiny red specks on top of one of the stools. Would blood in stool present like 2 small dots and nothing on the rest of the poop (I tried to take it out and spread it in tissue and came to no conclusion tbh)? What does blood in poop look like normally? My period also started yesterday (the very, very beginning of it, when there is just some mucus with a couple drops of fresh blood).

  • Blood in stools can present like fresh red per rectal bleeding/ maroon clots of blood,/ black coloured stools/streaks of blood on stools.
  • Isolated red spots in normal stools are not blood, because it usually mixes with the stool.
  • I do not think so; this is linked to your recent periods, too.
  • Avoid red coloured foods and spices.

Hello doctor,

I had no red on wiping or on the water. Was 2 spots close together on top of the stool, and I saw it just because I kind of dissected it. So let me see if I understood. Blood in stool, if fresh red blood, would be like rectal bleeding, i.e., the water would turn red and/or there would be red covering quite a bit on the stool, or streaks of blood on the stool.

  • Are there any pictures you could share of this available online?
  • Would fresh red not be (was very red), just 2 spots with maybe 1or 2mm?

The spots seemed to be on top, and not in the stool. I had strawberries, watermelon, and tomatoes the day before. I have a completely normal BM this morning, without any red in it at all. If case of colorectal cancer, what is the blood like? Is it constant, or can it happen once and never again? I got worried with this after seeing that.

  • You have eaten strawberries, watermelon, and tomatoes all red, there would be no small undigested particles of any food item.
  • You can find a lot of pictures on the internet, but this seems to be your health anxiety.
  • Colon cancer can occur in any person at any point in time, including me. It does not mean I should be unnecessarily worried all the time
  • Avoid unnecessary stress, take a proper diet, and drink a good amount of water.
  • Focusing on certain unnecessary things will trigger significant health anxiety and a disturbed life without an actual issue.

  • No, blood wouldn't look like that, as you have explained in detail. Blood is liquid, and it mixes with stools. A small amount of blood mixed with stools, so no change in stool colour, a large amount of blood, like bleeding gastric ulcers, or the stool colour becomes dark black. Blood near the anal verge will present as fresh red colour, either in the form of drops of blood or streaks of blood on stools. Infection in the colon can cause disturbed stool consistency and a variable amount of blood in stools / If small spots of red colour, as you mentioned, are highly likely to be undigested small food fragments.
  • Your stools are soft and formed, that's great, it is reassuring too.
  • You are right about misinformation online, but I do not have the option of attachments at my end.
  • Unfortunately, talking about colon cancer, I would suggest submitting your stool for microscopic examination, known as a stool detailed report and stool occult blood testing.
  • Colonoscopy is the best investigation telling about colon cancer in detail.

  • Would not be a spot on top of the stool inside the water, right?
  • Don't have any of this, but what about bloody mucus?

  • I did a stool microscopy analysis, like 3 months or so ago, and it was all ok. But after seeing that thought something had changed, and not sure they will do it again this shortly.

Yes, drops of blood will be visible and will either stain the toilet paper or the water. It would not appear as a red spot on the surface of the stool or inside the stool. Mucus would be thick white or yellow secretions along with stools, and if there is blood, it will turn the mucus red or pink in color. However, stool consistency would also be disturbed most of the time.

I would suggest observing at the moment. Avoid red colored meals for a while and do not take too much stress, as this will induce gut disturbances due to health anxiety. The red spots are undigested food particles and can be tomato-related or anything else in the diet. Considering your age, reddish discharge related to monthly periods sometimes mixes with stools.

All the spots can be related to the same diet and are mixed with normal stools. There is nothing to worry about. Please avoid red food products specifically. A stool microscopic examination can also help clear your doubts.

Blood is liquid and always mixes with stools, leading to discoloration such as black stools.

Too much blood leads to clot formation, presenting as maroon stools or fresh red blood.

Isolated spots with normal stool color are highly unlikely to be blood.

If there is a problem with the distal gut, such as the rectum or anal verge, one can see fresh blood or streaks of blood on stools, and sometimes mucus and blood mixed. Isolated spots with no mucus, normal stool color, and no other symptoms such as anal verge pain, swelling, or discomfort are very unlikely to be blood. Avoiding red foods for a couple of days will help greatly in differentiating.

I did eat tomatoes this morning again, but I will not eat tomatoes for the rest of the week and see what happens. Hopefully, it is just food. After talking to you, I am a bit more relaxed that this is most likely just food, as you said blood is liquid and would not appear as a piece of something on the stool. A clot would be darker if I understood correctly.

I do sometimes see a tiny bit of mucus on my stool when I am a bit more constipated or when there is a lot of poop. But there is no blood; it is yellow and transparent. I have been told several times that this is all normal, and that the problems would arise if there was a lot of it, if there were other symptoms, or if it had blood in it. Is that correct?

Just to double check, what I should worry about is: bright red streaks outside of the stool, such as mucus with blood, black stool, maroon stool or maroon bits, red toilet water, or bright red blood on paper. It would absorb into the paper and not appear as pieces.

Could you please have a look at the attached pictures and let me know what you think? I have had this since my child was born. It does not hurt; I do not even feel it except when I wipe after a bowel movement.

In rare cases, it can get a bit irritated with some stools and feel a bit like it has been scratched. It gets like that after a bowel movement and is a bit less swollen in between bowel movements. My general practitioner has been telling me it is a skin tag, but I am really scared it is a rectal prolapse. It is never red, and I do not feel like I am sitting on a ball.

I will answer everything.

I doubt it is external hemorrhoids because after a bowel movement, it gets less swollen almost immediately. It also does not bleed or cause any pain at all. It is like a piece of skin, though fatty skin, that comes out and then just a little bit is left out. If I stretch the area to the sides, it gets flatter like a piece of skin.

My child is three years old.

Yes, I did have an episiotomy. My child was born with the help of forceps.

Not at all. I have never had continence issues or lack of control.

There has been no size change at all. It has been stable for the past three years. It was much harder and redder when my child was born.

I did ask the doctor to have a look at this year. They even did a proctoscopy. I was told that my anal tone was very good and there were no lumps on palpation. The consistency, which is the bit I can feel between bowel movements, is like skin. After a bowel movement, it is a bit more fleshy and swollen. Should I be worried?

That is not symmetrical either. It may appear so, but when the skin is stretched, the area also stretches and there are parts without any lumps. I suppose that if it were cancer, it would have grown considerably and probably bled over the past three to four years. As far as I can tell, it has remained almost the same.

Are cancers in this area usually hard, firm, solid lumps? This feels very fleshy, almost like soft muscle or fat.

Apart from my General Practitioner (GP), who performed the proctoscopy, my gynecologist (gynaecologist), whom I see annually, has also examined this. I assume that if there were any suspicion of cancer, it would have been communicated.

  • Is that the reason for asking about fecal incontinence? I was under the impression that anal cancer appears more reddish in color, similar to a condyloma (wart-like growth).

Gets better on its own. 2) Once there is diarrhea, it resolve slowly, you may have semi solid / partially hard type of stools and then will go back to your normal routine. Last picture which you have shared , has formed stools already, so it will improve more. 3) Once you will empty all get, still there is normal gut secretions aswell. 4) Periods also causes abdominal pain and disturbed bowel to some extend 5) So to sum up, stay relaxed, eat clean food, probiotics are recommended for healthy gut , take good care of yourself wishing you best health. kind regards

Maximum water absorption occur in rectum, so if stool will stay longer in rectum, it will be more formed or even hard (incase of too much time like 2-3 days - hard constipated stool, initially followed by relatively soft stools) 2) similarly, if you go to toilet more frequently, stool will not stay in rectum for longer period and it will be semi- solid. Thank you. 2 questions then: 1. Is it normal to happen sometimes then. But why does sometimes and not always? I normally go 2-3x a day but habe to admit today was all in the space of 4h. When for colon cancer it is said "persistent changes in bowel movements" is it for example diahrrea that stays and never goes back to normal? Or csn be diarrhea then normal? There are many patients around , who have been diagnosed with certain type of cancers without any symptoms even. 3) Now coming back to gut , there is no hard and fast rule for colon cancer , recurrent diarrhea or persistent constipation or mixed symptoms. It can present in any form. If a person is totally asymptomatic, still there is a possibility he or she might develop cancer anywhere in the body including gut 4) you are thinking too much, medicine is not black and white or 2+2 is 4. This needs combination of symptoms, relevant investigations and clinical experience to make final assessment. 5) Gut is controlled by (subconscious mind) automatic nervous system, which is influenced by our thinking process and reacts accordingly. thanks for asking again So, 1) Persistent means , abnormal bowel habits (either semi-solids to loose stools or unusual constipation which is new onset ) for more than a month. ( incase of loose stools only/ we label it persistent diarrhea if 2-4 weeks and chronic,if more than 4 weeks) For constipation, new onset conditions for couple of weeks should be evaluated. 2) All invasive procedures will be risk Vs benefits, alternative to colonoscopy is CT scan colonography (no sedation and no insertion of tube in gut ). 3) You have significant health anxiety. You are paying undue attention to minor things , which we can't control as a human. To calm down your anxiety, either you have to ignore minor issues and give your gut some time to be regular or consider procedures as mentioned above Wishing you excellent health best regards

Have been now for a week (and one more week to go). So 2 days ago I had a type 6 stool brownish yellow (no other symptoms), yesterday and today was more formed (type 4/5ish) but when I wiped the paper was pretty yellow smeared. The stool was brownish/yellow. Today after 2 BM the 3rd BM was again a mix of type 5 and type 6 and a mix of brownish pieces and yellow (it was even leaking a bit yellow to the water today). Is this anything to worry about? I feel fine maybe a tiny bit gassy today but other than thay nothing else. The stools do not float, maybe the smell was a bit more pungent in all BMs today but nothing too crazy (have jad this in the past too). Is this concerning?

Hello, thanks for asking again so, I will explain don't worry. 1) Bile ( yellow colour) is normal production of liver cells and it flows from the liver and get stores in the gall bladder ( not produced in gall bladder, that's the reason why, when people operated for gall bladder removal, they don't have issues with their digestion later on). 2) This is same bile which come to small intestine via ducts and help in digestion of food, this bile have yellow pigments, which mixes with food contents, help their digestion and ultimately give colour to our stools. I have seen the picture you attached, there was nothing abnormal. 2) Don't concentrate too much on your stool colour, it is not in our control, black, maroon or fresh red blood should always be noticed. 3) Greasy stools mean, too oily stools, even you can feel like oil, difficult to wash and difficult to flush out Wishing you excellent health Best regards

Hi doctor, Thank you for the reply. I guess very pale stools are also to worry about but I guess to get to that point would be extremely jaundiced (and probably my livre blood testes would be totally messed up). There is something here that worries me a bit together with this though. You probably do not remember but I started having all this concerns after I took some probiotic that somehow messed me up for a long time (probiotics taken 6 months ago). I had messed up bowels for 2 weeks and I started burping more frequently too. The bowels got back to what I would say is normal but the burping took a very long time to resolve (months!) and a lot of stomach gurgles (which took about 2 months to go away too). The burps are mostly tasteless. During this burping period right after the probiotics I also had to take some antibiotics for tonsilitis which messed massively my stomach. I have tried lansoprazole because my GP thought that I had GERD (never had burning anywhere), did a H Pilory breath test (negative). I also had an increased mucus in my throat at some point. I visited a GI that at that point thought was probably stress and some dysbiosis. With time the burps started reducing and went to normal levels. It happens that I went to my home country the last 2 weeks and the burping re-started (I guess this is not anxiety now). I am a bit at a loss here and do not know what to think what do I have exactly. Can it be cancer (pancreatic, stomach, esophageal)? A food intolerance? What do you think I should do? I am attaching my recent blood tests too (from last week, they are in a different language but similar enough words I think) maybe that helps with something. Thank you

So, 1) I have reviewed the probiotics which you had taken , I have reviewed all attached reports including Complete blood profile, blood coagulation, Liver function tests, blood lipid levels, Thyroid function tests, blood protein analysis etc. 2) Your all reports were normal, nothing significant to explain your symptoms like frequent burbs etc 3) I haven't seen abdominal scans like ultrasound abdomen etc (to see liver and gall bladder specifically) 4) Sensitive stomach is one of the possibilities in your case, considering overall sensitive personality, this means, small triggers can upset your stomach and the more you will concentrate on it, the more it will be symptomatic. 5) Regarding your esophageal or stomach cancer like concerns , endoscopy is the best possible way to rule this out ( chances of tumor in your case, is exactly the same like me or any other normal individual) 6) I think, you should take acid suppression medicine like tab Lanzoprazole 30mg half hour before breakfast for 2 weeks and gaviscon 10ml post meals , for your symptoms and you will feel better. Hi Doctor, Noone sent me for a abdominal scan and they will not. I tried. Do you think is very important and concerning? Namelly since I got better at some point? Thank you. I suspect about intolerance also because I also have increased phlegm in the throat. Also I don't drink milk. I fo est cheese and butter and occasionally yoghurt. But on my 2 weeks away I was having a 300ml yoghurt everyday first thing in the morning. I have no pains anywhere. When burping is bad I feel a bit of pressure on my chest (no pain just uncomfortable). I have no issues eating nor lack of appetite. Burping has no taste at all. Burping gets better when I lay down to sleep mostly. I just cannot puzzle why did this start when I took the probiotics and everything got unregulated. I am not on lansoprazole atm, I was when all this started for a month and saw no changes at all. I also tried to take rennie after meals today and did not see much difference. Do you know slippery elm? Can give that a go maybe? Also wondered gallbladder issues but I have no pain, fats do not affect (burping can happen even with water) and according to my mum which had her gallbladder removed last year urine gets very dark and you feel horrible. So, 1) Temporary intolerance due to stomach upset is the possibility, but it will go away, it can take longer in over sensitive individuals 2) Your investigations and symptoms, both are ok( I mean no red flags , no pain, no major issues like bleeding, weight loss and family history of cancer etc) 3) Try to focus less on burbs and it will start improving 4) instead of Lanzoprazole, you can try tab Pantoprazole 40mg half hour before breakfast for 2 weeks, if you feel your burbs are too disturbing for you 5) slippery elm is sort of herbal medicine I think , I don't have any experience with that 6) Your LFTs are normal, less likely an active gall bladder issues, also there is no pain, which is agreed gall bladder issues too. Once you will be fine , you can have meals once in a blue moon like any normal individual. 2) Rifaxamin is locally acting antibiotics ( not absorbed in blood), keep be in gut and kill bad bacteria and go away in stools. This works good in bloating, changing bowel habits and in sensitive gut too , not specific for burbs. 3) You can try it , if gut related issues. Thanks for asking. That's why I switched PPI from Lanzoprazole to Pantoprazole, it doesn't mean, you didn't respond to one so you will never respond to other. Dysbiosis/ SIBO for both rifaxamin is good option. If you want to try it, that's relatively good and safe medicine. You can use this medicine for 7 days no issues kind regards

Pantoprazolw is not over rhe counter. Omeprazole is thoufh. But rennie and gaviscon also doesnt help if anything makes everything worse. I may try it just because this all started with the probiotics so I may have some sort of dysbiosis. I dont think is SIBO becquse I have no bloating. Just burping and my poop used to be a type 2/3 and now is a 4 and most of the type a 5. Do you think is safe and will do no harm and worst case scenario does nothintor should I pau 1000 for the breath test? My burps are nostly dry. No liquid, no taste, no smell. Thanks So, 1) Well, In that case , I don't think so using same medicine ( which you have already tried), will help you out. 2) Yes I agree, I don't think so you have SIBO, but Dysbiosis is a possibility and Rifaxamin is one of the safest medicine (I mean, this is one of its kind type of medicine, we don't have exact alternate of this medicine, this is the only one available for this purpose) so you can try it for good 7 days ( usually we give for 2 weeks ) 3) Your concerns are valid that what is going on, but to be honest stomach is way to sensitive to our thoughts and it will keep bothering you till you will be paying extra attention to it ( try to ignore burbs for couple of days and you will feel the difference :-) 4) I will email them, it shouldn't be like that. Hello doctor, I do have a related questions after some weird things yesterday. So yesterday night I had a pretty heavy meal. After the meal I bended over and felt like I needed to burp so I tried to push the burp but had to do some pressure, when I did this some “vomit” came to my throat and I cleared it to my mouth. I guess what happen here was that I made pressure in my stomach and forced a bit of content up together with bend over stomach position did not help. I did not burp or anything in the end. The weird thing was that I felt the liquid in my throat, but there was no burning or heartburn. I was absolutely fine. I drank some water to try push contents down and was all ok after. This got me a bit worried if my burps are actually bringing food / acid up and I have no clue about it because I do not feel it? I am starting to get really scared I have GERD or gastritis and all this acid is causing me Barrett’s oesophagus or even ulcers and I have no clue about it. Is this possible? Should I be worried? What should I do? I am afraid of burping and doing this again now. Everyone keeps telling me that heartburn is impossible to miss as it burns loads. But I am starting to get slightly worried. Any thoughts? The weird bit here too is that when I lay down in bed I get better. The burps stop (sometimes I get a bit more air in the other end but nothing major), and I feel just fine. So, 1) Burbs after eating heavy meals, can cause some simple regurgitation of food, that is pretty normal. 2) The food which you consumed a while back should not be that much acidic as I didn't mix with acid that much and as stomach is already full so it is not immediately contracting. Food takes 2-4 hours to leave stomach and go to small intestine 3) Barrett esophagus is very long term complications of reflux like more than 15-20 years of reflux, not few weeks or months of reflux can cause Barrett esophagus 4) I think, there is nothing to worry , just take normal meals and don't fill your stomach at once Wishing you best health kind regards

A couple more questions. today I have been having a horrible pressure feeling in my epigastric region. like a stuck burp that actually gets better after a burp and not sure what is going on. Hello, thanks for asking again So, 1) Immediately after meals, within minutes, if you burb and there is regurgitation of food , that is not that acidic, because there is no digestion of food within mins and it taste almost the same, may b mild acidic sometimes 2) Point 2, explanation is- People get heartburn and in fact a bad heartburn after heavy meals , but that is not immediately after meals , may b half hour or 1-2 hours after meals, that's why usually people have a very bad night after eating out or eating too heavy. Immediately after meals, there might be regurgitation of food, but less likely true acid reflux 3) In your case, stomach is working good that's great, nothing to worry.
