Hello doctor,

I have a problem with my lower leg (below the knee); I feel restless in my leg all the time. I want to visit the market, and anything after that I feel my leg has been very tired and needs rest, but when I go to rest I also feel that something happened in my leg and I have to move from time to time.

Please advise me on some tests to find out the actual problem, because I have taken many multivitamins and that problem is 10 to 15 years old. Because of this problem, I feel very lazy, avoid traveling, and I cannot stand for a long time.

Some suggest an MRI scan; some suggest a DEXA scan for leg bone. And also some suggested me a specialist for this type of problem.

I am 37 years old with a weight of 85 kilograms and height of 5'10" and my other reports are normal.

I assure you that not to worry at all, as things may get settled with proper care and follow-up. This kind of discomfort usually is due to either ischemia of the vessels of the legs or due to anemia (low hemoglobin levels). In both cases, the blood oxygen supply to tissues and muscles of the leg becomes deficient and causes ischemic pains. You need rest after some work. It shows that there is less supply of nutrition and blood to the muscles. But the accumulation of lactic acid in the muscles also causes discomfort when you start to rest.

Final suggestion:

Consult an Internal Medicine Specialist for Doppler of the legs, complete blood counts with differentials and a complete heart evaluation, and get the body's vessels checked for any atherosclerosis or arteriosclerosis. Do not smoke, do not take alcohol as it is poison for you.

Well, coming to your labs. It contains some important screening tests.

Regarding the one for diabetic screening, it shows totally normal values indicating good sugar control in your blood for the past four months. But to screen a person for diabetes in the first place, I recommend fasting blood sugar levels done from a laboratory. I highly suggest you to please go for that test so that I may decide conclusively that the health issue you have are not due to diabetes or prediabetes.

Also, go for a complete blood count with differentials; fasting lipid profile, liver functions tests and kidney function tests, thyroid profile, and serum electrolytes, and attach the reports along with the fasting blood test. Once I have these reports with me, the reason behind your illness will be clear. It is not an issue that cannot get treated.

The other lab result you attached is less sensitive and less specific for rheumatoid arthritis and is in the normal range. ACCP (anti-cyclic citrullinated peptide) is 0.81, but it is nonsignificant when correlated clinically.
