Hello doctor,

I am a 28-year-old male concerned about a condition in my testicles, which has persisted since my teenage days for about 14 years. They retract from my scrotum to my groin, and I have to push them back with my hands into the scrotum. How serious is my concern? Kindly suggest.

Are both testicles affected or only one? When you pull them down, do they go all the way down? Are you married or have children? Do you have adequate hair on your chest, axilla, and groin? Thank you for the reply.

I have had this concern since puberty. It is painful when I push them down. Both testicles are affected. Yes, they go all the way down when pushed, but only in some situations, like the cold. They go back to the groin even after pushing them into the scrotum. I am unmarried. I have underarm and groin hair sufficiently, but less on my chest. The size of my testicles and scrotum is small.

This condition is called retractile testis. It will not cause you any problems in terms of fertility or sexual activity. It is because your cremasteric muscle is overactive. I suggest you regularly check your testicles (once a month) to make sure they come down. If at any point later in life you feel that they are not coming down, you may need to see a urologist for a procedure called Orchiopexy, where the testicles are fixed to the bottom of the scrotum with absorbable sutures. There is a better chance that they remain as it is and cause no harm or discomfort.
