Is a black ulcer and severe swelling typical with scrub typhus?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a tea garden worker and have been diagnosed with scrub typhus. I have developed a black ulcer on my leg, and my fever has not subsided even after five days of medication. My whole body is swollen, and I have painful lumps in my neck and armpits. I have been coughing up blood since this morning and having difficulty breathing. My platelets are very low (45,000), and my liver enzymes are elevated according to the local clinic. Other workers in my section are also experiencing similar symptoms. Should I seek treatment at a larger hospital?

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I am sorry to hear you are having such a tough time. Scrub typhus is a serious infection, and the symptoms you are describing black ulcer on your leg, fever not responding to treatment, body swelling, painful lumps in your neck and armpits, coughing up blood, and difficulty breathing are concerning. The low platelet count (45,000) and elevated liver enzyme levels suggest that your condition may be worsening, which requires urgent medical attention.

Since other workers are also getting sick with similar symptoms, this suggests a potential outbreak, which makes prompt, specialized treatment even more critical. Scrub typhus can sometimes lead to complications like organ failure, bleeding, and respiratory distress, especially with low platelet counts.

Given your severe symptoms, it is highly recommended that you seek immediate medical care at a hospital or another major healthcare center. You may need advanced treatment, possibly including oxygen support, blood transfusions, and intensive monitoring. It is better on the side caution and get treatment from a hospital with experience handling such infections and complications.

Please do not delay in seeking professional medical care, as your condition requires urgent intervention.

Please let me know if you want some help.

Thank you.
