When to seek medical help for scrub typhus infection?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 25-year-old veterinary technician and have never been this ill before. I recently returned from volunteering at a wildlife sanctuary in India and have developed concerning symptoms. I have a black, crusty lesion behind my ear, a high fever with severe chills causing my whole body to shake, and extreme fatigue to the point that I feel like fainting when walking to the bathroom. My joints are painfully inflamed, and I experience frequent chills that make my teeth chatter.

The emergency clinic mentioned the possibility of scrub typhus. I am worried because I have animals depending on me at the shelter, but i am too weak to lift my arms. What should I do next?

Kindly help.

Hello,

Welcome to iCliniq.com.

I understand your concern.

This presentation suggests scrub typhus, given the eschar (black crusty lesion), high fever with shaking chills, severe joint pain, and extreme weakness with near-syncope (fainting). Possible complications include sepsis, myocarditis, and organ dysfunction.

Immediate Treatment

  1. Doxycycline 100 mg twice daily for 7–10 days (first-line).

  2. If allergic, Azithromycin is an alternative.

  3. Hydration is essential. If too weak to drink or vomiting, IV fluids may be necessary.

  4. Monitor for organ involvement

  5. Complete blood count (CBC) weekly to check platelets.

Liver function test (LFT) and kidney function test (KFT) weekly to assess liver and kidney function.

Electrocardiogram (ECG) and chest X-ray if chest pain, tightness, or breathlessness occur.

Magnetic resonance imaging (MRI) and lumbar puncture if confusion, dizziness, or blurred vision develop.

Seek immediate medical attention if you experience

Severe dizziness, fainting, trouble breathing, chest pain, confusion, seizures, difficulty waking up, very dark urine, or no urine output. Scrub typhus can be fatal if untreated. Seek help immediately and arrange for coverage at work.
