Does compounded Semaglutide cause extreme fatigue and constipation?

Hi doctor,
I recently started compounded Semaglutide from a wellness clinic. I am 61 years old with a BMI of 37, hypertension, prediabetes, and osteoarthritis, making exercise difficult. I have been on it for three weeks at 0.25 mg weekly with some concerning side effects. I am now experiencing extreme fatigue, severe constipation (no bowel movement for five days despite laxatives), and persistent headaches. I lost eight pounds already, which seems excessive in a short timeframe. Are these side effects normal or signs of impurity issues? Also noticed a black tarry stool, which is concerning. The clinic wants to increase my dose to 0.5 mg next week, but I am hesitant given the side effects. Should I request testing to verify what is actually in the medication? What safety questions should I ask the compounding pharmacy?
