Hello doctor,

I have been concerned with my skin lately. There were some strange (I do not know if they are pores) growing bigger and expanding through my arms. I noticed they are only located on my arms, and nothing is found on my legs or anywhere. I do not know exactly when they appeared. I just noticed them last week. I have tried putting some moisturizer on the affected area, but I cannot see any changes. I also washed my skin with baking soda paste and even apple cider vinegar to subside irritation. Is this just normal since the weather in our tropical country is between 30 degrees Celsius? But I want to vanish it before the first day of class starts. What should I do? Please help.

On examining your pictures (attachments are hidden to protect the patient's identity), it seems you have PMLE (polymorphic light eruptions). PMLE is idiopathic, sunlight, or artificial light-induced cutaneous eruptions, ranging from papulovesicular lesions to large plaques, mainly localized to the photo-exposed areas of our body. Suggested treatment includes:

1. Avoid midday sunlight exposure.

2. Restrict the use of broad-spectrum and high-protection sunscreen.

3. A short course of topical steroids like Mometasone.

4. In the event of repeated attacks, a prophylactic course of low-dose PUVA therapy (phototherapy) using 311 nm UVB (ultraviolet-B) is the treatment of choice.
