Patient's Query
Hi doctor,
I am having joint popping and a low platelet count with LFT abnormalities. Please help as per the report.
What is the reason for all these?
I am taking the tablet Minoxidil for a hair problem.
Kindly suggest.
Thank you.

Patient's Query
Hi doctor,
Thank you for the reply.
I am not able to find the write-up.
Please help.
Thank you.
Hi,
Welcome back to icliniq.com.
Low platelet count, also called thrombocytopenia.
Grade 1 thrombocytopenia: The platelet count ranges between 75,000 and 150,000 platelets per microliter of blood. No action is needed. Just follow up.
Grade 2 thrombocytopenia: The platelet count ranges between 50,000 and 74,000 platelets per microliter of blood. Do not panic. Investigate the cause. The top-of-the-line cause is usually nutritional deficiency, chronic infections, or some drug-induced destruction.
Grade 3 thrombocytopenia: The platelet count ranges between 25,000 and 49,000 platelets per microliter of blood. Do not panic about the platelets. Panic about the cause. Possible causes might be septicemia, ITP (idiopathic thrombocytopenic purpura), and bone marrow infiltration.
Grade 4 thrombocytopenia: The platelet count ranges between 5,000 and 24,000 platelets per microliter of blood. Need to panic about platelets and the cause of both. However, no platelet transfusions are needed. Needs frequent monitoring of the patient. Causes include acute ITP, septicemia, DIC (disseminated intravascular coagulation), TTP (thrombotic thrombocytopenic purpura), acute leukemia, and aplastic anemia.
Grade 5 thrombocytopenia: The platelet count falls below 5000 platelets per microliter of blood. It is the critical level of platelets. Needs hospitalization and immediate platelet transfusion. However, Eltrombopag is superior to platelet transfusion in most cases. The causes are the same as those of grade 4 thrombocytopenia.
I hope this helps.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
When you say "follow up for Grade 1," do I need to repeat the test?
And I forgot to inform you that I had a second dose of Covaxin three weeks back.

Hi,
Welcome back to icliniq.com.
There is no need for follow-up.
Do a CBC (complete blood count) test after a couple of months.
Just monitor the signs and symptoms of bleeding (petechiae, bruising, epistaxis, gum bleeding, hematemesis, hemoptysis, bleeding per rectum, and menorrhagia are signs of bleeding).
Vaccines are also a cause of mild to moderate thrombocytopenia.
I hope this helps.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
I have had gum bleeding problems for the last five to seven years. After that, I did CBC around five years and eight years ago. Those reports looked normal with respect to platelets.
Hope there is no need to worry?
Please help.
Thank you
Hi,
Welcome back to icliniq.com.
It is good to see that all reports are normal (attachments removed to protect the patient's privacy).
There must be a local dental cause of bleeding.
Even at platelets of 112,000 per microliter of blood, there will not be any active spontaneous bleeding.
I hope this helps.
Thank you.
