What’s the best multidisciplinary care for T1D at age 35?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 35-year-old type 1 diabetic with multiple associated conditions. I have been managing diabetes for 20 years and recently developed hypothyroidism and early-stage diabetic retinopathy. My current insulin regimen includes Novolog and Levemir, with a total daily dose of 45 units. My latest comprehensive metabolic panel shows early signs of kidney stress, with microalbuminuria at 40 mg for 24 hours and eGFR slightly reduced to 68 mL/min/1.73m². I am also managing hypertension with Lisinopril and experiencing occasional peripheral neuropathy symptoms. My doctor recommended a multidisciplinary approach, but I am seeking more detailed guidance on managing these interconnected health challenges.

Please help.

Hello,

Welcome to icliniq.com.

I read your query and understand your concern.

Managing your multiple health conditions requires more care to prevent complications. For diabetes management,

  1. Aim for tight glucose control through your current insulin regimen, considering continuous glucose monitoring (CGM) to fine-tune insulin doses.

  2. For hypothyroidism, thyroid hormone replacement is needed to optimize metabolic control.

  3. For kidney health, continue monitoring microalbuminuria, RFT, Urine complete, and eGFR, and ensure good blood pressure control with Lisinopril, which also helps protect your kidneys.

  4. For peripheral neuropathy, make sure B12 and folate levels are normal, and plan for nerve-protective agents like gabapentin, and regular exercise can improve circulation and reduce neuropathic symptoms.

  5. Regular follow-ups with your endocrinologist, nephrologist, and cardiologist are essential for coordinated care.

I hope this helps

Thank you.
