Hello doctor,

My fiance and I had intercourse a few days ago, and by accident the condom broke and semen had entered inside. She immediately took an Unwanted-72 pill. We are very worried that this could lead to pregnancy, as we cannot afford it now. Please let me know about the possibility of pregnancy. She started having cramps a few days before, which means periods are about to come. So does this mean the fertilization window is over? As per a recent scan, her endometrial thickness is 6 mm.

She is having irregular periods, and her last period was two months ago. She did not have a period last month. Should we expect a withdrawal bleeding after taking the Unwanted-72? How do we know if the pill has worked? Please help, as we are very worried.

A lady with an irregular period, most times does not ovulate (the process by which an ovary releases an egg), so it is difficult to predict her fertility window (the period during a woman's menstrual cycle when she is most likely to conceive, usually around the time of ovulation). However, if her last period was about two months ago and she has not had another period since that circle started is an anovulatory cycle (that means ovulation did not take place and is not likely to take place). So, there is no chance of pregnancy occurring from the recent intercourse.

Also, unwanted-72 is effective at preventing pregnancy, so considering her menstrual history and the unwanted-72 taken, there is no chance of pregnancy occurring. Yes, expect withdrawal bleeding within the next seven to 14 days. It may come as bleeding, spotting, or brownish discharge. So, generally, there is no risk of her getting pregnant from the recent intercourse.

Withdrawal bleeding is an indication that the Unwanted-72 pills worked. However, it is not at all time you see withdrawal bleeding. This is because it depends on the endometrial thickness when the pill is taken. Endometrial thickness less than 8 mm mostly gets spotting, brownish discharge, or cramping some days after the Unwanted-72 pills. So, apart from these, there is no indication to show if it worked or not.

But, studies have shown that even without a withdrawal bleeding effectiveness is still very high. So, if there is no withdrawal bleeding, there is nothing to do. The earliest time you can do a pregnancy test is three weeks (provided no period or withdrawal bleeding) from the date of the sex as most pregnancy test kits are sensitive then.
