Patient's Query
Hello doctor,
I am 40 years old and have been diagnosed with metastatic urothelial carcinoma. My treatment plan includes possible surgery, but my doctor mentioned starting chemotherapy before surgery.
I would like to understand whether chemotherapy before bladder surgery is necessary in my case.
Does pre-surgery chemotherapy significantly improve outcomes? I am trying to understand the treatment sequence and why it is planned this way.
Please help.
Hello,
Welcome to icliniq.com
I have read your query and understand your concern.
In metastatic urothelial carcinoma, the sequence of treatment is very important. Chemotherapy is often given before surgery, a strategy known as neoadjuvant chemotherapy, to help control disease that may have spread beyond the bladder.
This approach can help shrink the tumor, reduce microscopic disease, and improve the chances of better outcomes when surgery is performed later. It also allows doctors to assess how the cancer responds to treatment.
In cases where the disease is already metastatic, systemic therapy such as chemotherapy or targeted therapy is usually the main initial treatment, as surgery alone may not be sufficient to control the disease. Once the cancer is better controlled or stabilized, surgery may be considered in selected situations.
Starting with surgery without controlling the disease first may lead to less effective outcomes, which is why chemotherapy is often recommended as the first step.
The exact plan depends on factors such as the extent of spread, overall health, and response to treatment. Your oncology team will tailor the approach to achieve the best possible results.
I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.
Thank you.
