What could cause pain while using a vacuum erection pump?
Patient's Query
Hello doctor,
I am a male, mid-30s. I started using a vacuum erection pump last month. After 10 days, I began experiencing mild pain and cold sensation at the tip (glans) of the penis, especially in a flaccid state.
Current symptoms:
- Cold or slightly numb feeling in the glans (intermittent).
- Occasional mild pain or soreness in the shaft and tip.
- Penis feels dry and mildly swollen at times.
- Morning erections occur but are not fully firm.
- No issues with urination, no burning or discharge. I can feel touch across the entire penis.
Main concerns:
- Possible nerve or blood flow issue.
- Whether this could lead to long-term damage.
- Guidance on recovery and whether an in-person examination is needed.
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I can understand your concern.
It is reassuring to note that your penile exam appears normal. Based on your symptoms and timeline, this sounds like a case of mild tissue sensitivity and superficial nerve or vascular irritation following the use of a vacuum erection device (VED). This is not uncommon when first starting, and it is generally temporary and reversible.
What is likely happening:
Your symptoms- mild soreness, intermittent cold or numb glans, reduced firmness in erections, and some dryness, suggest:
- Venous congestion or mild lymphatic swelling from overuse or excessive negative pressure.
- Superficial nerve irritation affects the sensitivity and response of the glans.
- Minor dryness or reduced surface circulation, especially if moisturization has not been optimal.
The good news is:
- You still have a normal touch.
- You are getting morning erections.
- There is no pain during urination or significant color change.
All of these are reassuring signs.
What you should do now:
1. Discontinue pump use temporarily- Take a complete break for at least two to three weeks from using the VED or any vigorous sexual stimulation. This will give the tissues and nerves time to recover.
2. Local care- Replace olive oil with a more effective barrier or moisturiser like coconut oil, vitamin E oil, or plain petroleum jelly, applied twice daily. Continue gentle warm compresses once or twice a day if they help relieve discomfort.
3. Start a B-complex supplement- Choose one with B1, B6, and B12 to support nerve repair. You can safely continue ashwagandha.
4. Lifestyle tips- Stay well hydrated and avoid nicotine or excess caffeine, which can reduce peripheral circulation. Light exercise (like walking or stretching) helps improve blood flow.
Do you need to see a urologist in person immediately?
Not immediately, unless:
- Symptoms worsen.
- You lose sensation.
- Erections stop completely.
- You notice any dark discoloration, ulcers, or bleeding.
However, if symptoms persist beyond three to four weeks, a urology consult is advised. A penile Doppler ultrasound may be done to evaluate blood flow if needed. This does not appear to be serious or permanent. These types of minor injuries from vacuum pump overuse are very common and usually recover fully with rest and conservative care.
You are doing the right thing by addressing it early and being cautious. Let us reassess your progress in a few weeks. Once things settle, I would be happy to help you reintroduce the VED safely and gradually to prevent recurrence. Wishing you a smooth and full recovery.
I hope this information will help you.
Thank you.
