How can I treat my white foul vaginal discharge?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I would like to ask for your advice regarding a vaginal discharge concern that started around nine months ago. I have been experiencing a white, lotion-like discharge with a foul odor, but there is no itching or pain.

I also noticed that during my ovulation period, I no longer have the usual jelly-like or stretchy discharge I used to see.

Nine months ago, I had been using a Niacinamide soap that was gifted to me by a friend, but I stopped using it recently in case it affected my vaginal balance.

May I kindly ask if there is any medicine or remedy I can use to help with this condition?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

The type of discharge you are describing, like white, lotion-like, and with a foul odor but without itching or pain, sounds more consistent with bacterial vaginosis, or BV, rather than a yeast infection, since yeast usually causes itching and a thick, cottage cheese-like discharge.

BV occurs when the natural balance of vaginal bacteria is disrupted, which can happen after using soaps or products that alter vaginal pH (potential of hydrogen), so stopping the Niacinamide soap was a good step. The change you noticed in your ovulation discharge may also be related to this imbalance, since BV can affect the normal cervical mucus pattern.

The best approach is to confirm the diagnosis with a gynecologist, who can do a simple vaginal swab and, if BV is confirmed, prescribe antibiotics such as Metronidazole or Clindamycin in oral or vaginal forms. Over-the-counter (OTC) remedies are generally not effective for BV.

In the meantime, avoid scented soaps, douches, or feminine washes, and wash only with water or mild, unscented cleansers on the outside. Wearing breathable cotton underwear and keeping the area dry can also help maintain balance. Since untreated BV can sometimes increase the risk of other infections, it is best to get checked and treated properly.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
