Hello doctor,

I have been noticing yellowish discharge in my vagina, and sometimes my vagina hurts. It has been a month now. What should I do?

I understand your concern.

If there was unprotected sexual contact, we need to rule out sexually transmitted infections (STIs).

Please do the following tests:

1. TPHA (treponema pallidum hemagglutination).

2. Herpes simplex virus 1 and 2 – NAAT (nucleic acid amplification test).

3. RT-PCR (reverse transcription polymerase chain reaction).

4. Urine polymerase chain reaction for Chlamydia trachomatis and Neisseria gonorrhoeae.

5. Human immunodeficiency virus 4th-generation antigen/antibody screening.

6. Hepatitis B surface antigen.

7. Urine complete analysis.

8. Urine culture and sensitivity.

9. Complete blood count and erythrocyte sedimentation rate (ESR).

These tests will help us rule out common causes of genital ulcers, discharge, and irritation after exposure.

If it looks like a bacterial infection:

1. Apply Mupirocin ointment locally twice daily.

2. Doxycycline 100 milligrams twice daily for five days (after food).

If there is significant itching (suggestive of fungal infection):

1. Apply Clotrimazole and Beclomethasone ointment locally twice daily.

2. Cetirizine, one tablet at night for itching.

General genital hygiene advice:

1. Wash the genital area twice daily (morning and night).

2. Use normal room-temperature water.

3. Avoid hot water or cold water.

4. Do not use harsh or high-pH soaps.

5. Keep the area dry and avoid tight clothing.
