Hello doctor,
I am a 40-year-old woman and have been experiencing some vaginal dryness. Occasionally, there is an unpleasant odor, especially just before my menstrual period. Additionally, I have noticed that I have difficulty holding my urine for extended periods. Please help.

At 40, it is quite common to experience vaginal dryness and occasional odor, especially before your period. These symptoms are often linked to hormonal changes that occur during perimenopause, the transitional stage leading up to menopause. As estrogen levels fluctuate, the vaginal tissues may become thinner and less lubricated, which can lead to dryness and a slight change in odor.
Hormonal shifts can also affect the vaginal pH (potential of Hydrogen), making these changes more noticeable around menstruation. The difficulty in holding urine, known as urinary urgency or urge incontinence, can also be related to hormonal changes, weakened pelvic floor muscles, or mild bladder irritation.
To help manage these symptoms, staying well-hydrated, limiting bladder irritants like caffeine and alcohol, and doing regular pelvic floor (Kegel) exercises can be beneficial. For dryness, water-based lubricants or vaginal moisturizers can help. In some cases, your doctor may recommend an estrogen-based treatment.
However, if the vaginal odor becomes strong or if you experience itching, burning, or unusual discharge, it is best to see a gynecologist, as these may be signs of an infection like bacterial vaginosis or a yeast infection. A simple pelvic exam and urine test can help determine the cause and guide appropriate treatment.
