Hello doctor,

I had really rough sex multiple times two to three days ago, and also rubbed myself against a pillow after. Since then, my vulva has been super sore or burning, and it hurts to sit, pee, cycle, and even poop sometimes.

I noticed some swelling, tiny bumps, or pimple-like spots near the vaginal opening, and the skin feels raw or irritated. Also, there is no major bleeding or fever.

  • Does this sound like friction tears, irritation, or something else?

Your symptoms strongly fit:

  • Raw burning vulvar skin.

  • Swelling and tenderness.

  • Pain while sitting, urinating, cycling, or passing stool.

  • Small, inflamed bumps from friction or blocked, irritated glands.

This is commonly caused by excessive rubbing, dryness, prolonged intercourse, or lack of lubrication. What you can do at home:

  • Avoid sex, masturbation, cycling, and friction for several days.

  • Wear loose cotton underwear.

  • Wash gently with lukewarm water only.

  • Avoid soaps, scented products, wipes, or antiseptics.

  • Apply a plain protective barrier, such as petroleum jelly or zinc oxide, externally.

  • Cold compresses for 10 to 15 minutes can help reduce swelling.

  • Drink more water because concentrated urine can increase burning.

  • You may use Paracetamol or Ibuprofen if they are safe for you.

Healing usually starts improving within a few days and often settles within one to two weeks. The small bumps you noticed can sometimes simply be inflamed hair follicles or swollen glands caused by trauma and irritation.

However, if the bumps become blisters, ulcers, pus-filled, spreading, or very painful, another condition, such as infection or herpes, should be ruled out.

Please seek medical examination urgently if:

  • Fever develops.

  • Severe swelling occurs.

  • Pus or foul-smelling discharge develops.

  • Difficulty urinating occurs.

  • Worsening sores develop.

  • Symptoms are not improving after several days.

Acute vulvar friction dermatitis with superficial vulvar abrasions/microtears secondary to repeated mechanical trauma.
