Based on your history, symptoms, the location of the lesions, the image provided (attachment removed to protect the identity of the patient), and your past episode of jock itch, the findings are consistent with tinea cruris, which is a fungal infection of the groin.

Tinea cruris typically presents as itchy, white or reddish patches with scaling. It may also be associated with a burning sensation or pain, especially in warm, moist areas such as between the thighs and around the private parts. Given that the condition has persisted for a few months, it indicates the need for regular, adequate antifungal treatment rather than occasional or irregular application.

For treatment, I would suggest you:

  • Apply Bifonazole 1% cream twice daily. Apply a thin layer over the affected area, extending it approximately 1 to 2 centimeters beyond the visible margins of the lesion. This treatment should be continued consistently for at least three to four weeks, even if the itching or redness improves earlier, as stopping treatment too soon can lead to recurrence.

  • To help relieve itching, you may take Cetirizine 10 mg at bedtime for a few days. This can help reduce itching and improve sleep.

  • Along with medication, proper hygiene and care are equally important. Keep the affected area clean and completely dry after bathing. Gently pat the area dry with a towel, or use a cool-air setting if necessary.

  • Avoid tight underwear or jeans; instead, wear loose-fitting cotton undergarments and change them daily.

  • Refrain from scratching the area, as this can worsen the infection and may lead to secondary complications.

  • It is important to avoid applying steroid creams, combination creams, talcum powders, or home remedies, as these can aggravate fungal infections and delay healing.

  • Additionally, avoid sharing towels or clothing, as fungal infections can spread from one person to another.

Please remember that fungal infections take time to resolve. However, with consistent treatment and good hygiene, they usually clear well. If your symptoms do not improve after completing the full course of treatment or worsen, a follow-up evaluation may be necessary.
