Overview:

Children's and adults' outer ear infections are treated with Ciprofloxacin otic solution. It is used to treat acute otitis externa, an ear infection brought on by certain bacteria. It works by eliminating the pathogens that cause infections, mainly Pseudomonas aeruginosa or Staphylococcus aureus. The drug was approved by the United States Food and Drug Administration (USFDA) in 1987.

Drug Group:

Ciprofloxacin otic solution is part of a group of antibiotics called quinolones. It works by killing the bacteria that cause the infection.

Dosage:

Ciprofloxacin otic solution is available in liquid form. For seven days, apply one container (containing 0.25 milliliters) of the ear drops to the afflicted ear twice daily, separated by around 12 hours. Each dose has 0.5 milligrams of medication (0.2 percent Ciprofloxacin) and contains a sterile, preservative-free Ciprofloxacin solution.

Indication:

Acute otitis externa brought on by susceptible strains of Staphylococcus aureus or Pseudomonas aeruginosa can be treated with ciprofloxacin otic solution.

Contraindication:

People with a history of Ciprofloxacin hypersensitivity should not use Ciprofloxacin otic solution.

Why Is Ciprofloxacin Otic Solution Used for Acute Otitis Externa?

Inflammation of the ear canal is a common symptom of acute otitis externa, which is usually brought on by a bacterial infection. Staphylococcus aureus and Pseudomonas aeruginosa are the most prevalent pathogens. Otalgia (ear pain), itching, edema (swelling), erythema (redness), and otorrhea (discharge) are all signs of acute otitis externa, which is characterized by an abrupt onset of ear canal inflammation. The illness frequently develops after swimming or minor cleaning-related damage. Tenderness when the tragus or pinna moves is a classic symptom.

Ciprofloxacin otic kills certain bacteria by specifically targeting and interfering with the organism's capacity to produce and repair DNA (deoxyribonucleic acid).

The medicine should not be shared with other people, even if they suffer from the same conditions and symptoms. This medicine is available by prescription and hence, always consult a healthcare provider before using this medicine. How Should Ciprofloxacin Otic Solution Be Used?

Wash your hands before using Ciprofloxacin ear drops. Warm the container in your hands for at least one minute to lessen dizziness caused by a chilly solution. Place the drops in the ear while lying down with the afflicted ear facing up. To prevent the drops from going deep into the ear, hold this position for at least a minute. Repeat for the other ear if both are impacted. For seven days, use the drops twice daily at around the same time. Ask your doctor or pharmacist any questions you may have, and make sure you follow the directions on your prescription label. Follow the directions strictly; do not change the dosage or frequency without first talking to your doctor.

What Are the Side Effects of Ciprofloxacin Otic Solution?

Ear drops containing Ciprofloxacin may have adverse consequences. If you have any persistent or severe symptoms, like ear pain, irritation, or itching, get in touch with your doctor.

  • Inform your physician and pharmacist of any allergies you may have to Ciprofloxacin, Gemifloxacin, Levofloxacin, Lomefloxacin, Moxifloxacin, Norfloxacin, Ofloxacin, Nalidixic acid, or any of the other drugs or components in Ciprofloxacin otic.

  • All prescription and over-the-counter drugs, vitamins, nutritional supplements, and herbal products that you now use or intend to take should be disclosed to your physician and pharmacist.

  • Inform your physician if you are nursing a baby, intend to get pregnant, or are already pregnant. Contact your physician if you become pregnant while taking Ciprofloxacin otic.

  • Additionally, you should be aware that when taking ciprofloxacin otic, you need to keep your infected ear or ears dry and clean. Unless your doctor instructs you otherwise, avoid swimming and getting the infected ear or ears wet while taking a bath.

Missing Dose:

Apply the missed dose as soon as you remember. However, leave out the missed dose and stick to your usual dosing plan if your next dose is almost here. Never use additional eardrops to make up for a missing dosage.

Even if your symptoms get better or go away, make sure you finish the entire course of Ciprofloxacin otic. This lessens the possibility that infections in the future will develop resistance to Ciprofloxacin or comparable drugs.

Overdose:

Seek medical help at the earliest in case of overdosing.

Clinical Pharmacology

  • Chemical Name: 1-cyclopropyl-6-fluoro-1,4-dihydro-4-oxo-7-(1-piperazinyl)-3-quinoline carboxylic acid.

  • Molecular Formula: C17H18FN3O3•HCl•H2O.

  • Molecular Weight: 385.82.

  • Inactive Ingredients: Glycerine, povidone, and water for injection. To adjust pH, sodium hydroxide or lactic acid may be added.

  • Mechanism of Action: DNA gyrase is an enzyme necessary for bacterial DNA synthesis, and ciprofloxacin inhibits it to provide its bactericidal effect. Chromosome- or plasmid-mediated processes can lead to the development of bacterial resistance to quinolones. Ciprofloxacin and other fluoroquinolones work differently from macrolides, which means that ciprofloxacin may be able to target organisms that are resistant to macrolides and vice versa. Studies conducted in vitro have demonstrated that ciprofloxacin and certain other fluoroquinolones exhibit cross-resistance. In vitro and clinical cases of acute otitis externa, Ciprofloxacin has shown efficacy against the majority of isolates of the following bacteria (Aspergillus aureus - An example of Pseudomonas).

  • Pharmacokinetics: Plasma concentrations of ciprofloxacin were not assessed after 0.25 mL of Ciprofloxacin otic solution (total dose: 0.5 mg Ciprofloxacin) was administered. However, it is anticipated that the maximum Ciprofloxacin plasma concentration will be less than 5 ng/mL (nanogram per milliliter).

Non-Clinical Toxicology: Ciprofloxacin is given orally every day for up to two years, even at high doses; 750 mg/kg (milligrams per kilogram) for mice and 250 mg/kg for rats, has not been linked to cancer or tumors in long-term studies in mice and rats. To evaluate the potential of Ciprofloxacin otic solution to induce cancer, however, no long-term studies have been conducted on the drug.

Storage and Handling:

Ciprofloxacin otic solution is a sterile, colorless, and transparent solution that doesn't contain any preservatives. It is supplied in single-use low-density polyethylene (LDPE) containers as a 0.2 percent otic solution. A 0.25 mL solution containing 0.5 milligrams of Ciprofloxacin is delivered in each container. The product (NDC 66992-450-14) comes in a carton containing 14 single-use containers that are enclosed in a foil pouch. Ciprofloxacin otic solution should be kept between 15°C (degrees Celsius) or 59 degrees Fahrenheit and 25°C or 77°F. Once a container has been used, dispose of it. To shield unused containers from light, keep them in the bag.

Drug Warnings and Precautions:

  • Otic Use Only: Ciprofloxacin otic solution is only meant for usage in the ear region. Avoid using it for topical ophthalmic (eye) treatments, injections, or inhalations.

  • Hypersensitivity: If you experience a skin rash or any other symptoms of hypersensitivity, stop taking Ciprofloxacin otic solution immediately.

  • Long-Term Use Causes the Growth of Resistant Organisms: Like other antibiotics, prolonged use of Ciprofloxacin otic solution may cause non-susceptible organisms, such as fungi and yeast, to proliferate. Stop the Ciprofloxacin otic solution and start an alternate treatment if a superinfection develops.

Use in Specific Populations:

  • Pregnancy: Animal studies have demonstrated that Ciprofloxacin administered orally or by injection does not cause harm to the developing fetus in rats and mice. High oral dosages in rabbits did not result in birth abnormalities, but they did produce stomach problems and weight loss in the mother, which increased the frequency of miscarriages. Ciprofloxacin administered by injection did not cause harm to either the mother or the fetus. Studies on Ciprofloxacin otic solution, particularly in pregnant humans or animals, have not been conducted. Consequently, pregnant women should use Ciprofloxacin otic solution with caution.

  • Nursing Mothers: It is unknown whether ciprofloxacin is eliminated from human milk after otic usage. However, it is eliminated when it is used systemically. It should be determined whether to stop breastfeeding or stop taking the medication, taking into account the drug's significance to the mother, because nursing infants are susceptible to severe adverse responses.

  • Pediatric Patients: Although controlled clinical trials have shown that Ciprofloxacin otic solution is effective in treating otitis externa in pediatric patients aged one year and older, there is no evidence that otic administration of quinolones affects weight-bearing joints, even though systemic administration of some quinolones causes arthropathy in immature animals. Safety and efficacy in infants under one year of age have not been established.

  • Geriatric Patients: There are no obvious safety or efficacy differences between younger and older patients.

Clinical Studies:

Patients with acute otitis externa were randomized to either Ciprofloxacin otic solution twice daily or a Polymyxin B sulfates, Neomycin, and Hydrocortisone (PNH) otic solution three times daily for seven days in a multi-center, evaluator-blinded research.

Clinical cure rates were 70 percent (173 out of 247) in the Ciprofloxacin otic solution group and 60 percent (147 out of 243) in the control group at the completion of the seven days treatment in the per-protocol population.
