The heart and the lungs are two important organs of the human body. While the lungs help in the process of gaseous exchange the heart acts like a pump and maintains the blood circulation. As a result, the cardiopulmonary system is one of the most important systems of the human body. In daily activity and during physical activity the cardiopulmonary system plays a pivotal role. That is why understanding the wellbeing of the cardiopulmonary system is important.

What Is the Importance of Cardiopulmonary Assessment?

Understanding the cardiopulmonary system and its involvement during physical activity is important. The respiratory system undergoes adaptations during exercise by augmenting both the frequency and intensity of breathing, leading to increased ventilation to compensate for the increased oxygen requirements of the active muscles. This demonstrates the body's impressive capacity to respond to specific demands. In addition, vasodilation takes place in blood vessels near the working muscles, facilitating enhanced blood circulation and improving gas exchange at the capillary level. Subsequently, the heart rate increases to accommodate larger quantities of oxygen-rich blood across the body. So, the determinants of the physiological limits are;

This signifies the maximum potential of an individual to intake, carry, and utilize oxygen. It characterizes the functional aerobic capacity of that person. Cardiopulmonary exercise testing, or CPET or CPEX, is a diagnostic procedure utilized to assess exercise capacity. This specialized stress test is also referred to as the oxygen consumption test. This helps to provide information on the heart and lungs to evaluate the body's physiological response to physical activity, determining whether it is within normal parameters or indicative of abnormalities. This is a gold standard test for assessing cardiorespiratory fitness and exercise capacity.

What Are the Indications of Cardiopulmonary Exercise Testing?

Cardiopulmonary exercise testing is done in the following conditions;

  1. Determining the causes of unexplained dyspnea or shortness of breath.

  2. To determine the exercise capacity.

  3. To evaluate the risk of cardiopulmonary complications during a surgical procedure.

  4. It is a useful test for assessing lung conditions in the post-treatment assessment of disorders like lung cancer, heart or lung transplant, and lung volume reduction surgery.

  5. This test helps to determine exercise patterns for pulmonary rehabilitation.

  6. Assess the severity of respiratory disorders, including chronic obstructive lung disease, pulmonary vascular disease, cystic fibrosis, and various other conditions.

What Is the Procedure for Cardiopulmonary Exercise Testing?

The exercise is typically performed on a stationary cycle ergometer or a motorized treadmill, with a focus on reproducibility and patient safety. Both options offer increased resistance to measuring a patient's work output in watts over time. However, maximal oxygen uptake is often higher (5 to 20 percent) on a treadmill compared to a cycle ergometer due to the engagement of more muscle groups. Additional equipment such as a metabolic cart, gas analyzer, computer, and screen displaying 12-lead EKG analysis and physiological changes during exercise are also used. The participants should keep the following things in mind;

  1. Wear loose and comfortable clothes during the process.

  2. Avoid any exercise or heavy physical activity on the day of the assessment.

  3. Avoid smoking, vaping, or drinking coffee on the day of assessment.

  4. Avoid heavy foods on the day of exercise. Also, refrain from consuming any food for at least three to four hours before the examination.

At the beginning of the procedure, different pulmonary function tests, such as spirometry, are performed. For this procedure, EKG (electrocardiogram) leads are attached to the chest. In the upper portion of one arm, a calf monitor is attached to the blood pressure. A pulse oximeter in the hand and a mouthpiece or mask are also used. Within this mask, a flow meter with tubing is attached for breathing.

At the beginning of the procedure, different pulmonary function tests, such as spirometry, are performed. For this procedure, EKG (electrocardiogram) leads are attached to the chest. In the upper portion of one arm, a calf monitor is attached to the blood pressure. A pulse oximeter in the hand and a mouthpiece or mask are also used. Within this mask, a flow meter with tubing is attached for breathing.

The test starts with two to three minutes of warm-up. The optimal duration for functional assessment on a bicycle is between 8 to 17 minutes. The workload on a bicycle is measured in watts (W) or kilopod meters per minute. Also, electronically braked bicycle ergometers can be used with a ramp protocol. The modified Naughton protocol is recommended for treadmill exercise testing in patients with heart failure. This protocol aims to increase the workload every two minutes. Treadmill exercise testing offers several advantages over cycle ergometry, and for most individuals, walking on a treadmill is a more familiar activity than cycling. It involves a larger muscle mass and requires more work against gravity. As a result, the peak oxygen consumption (PVO2) is, on average, five to ten percent higher on the treadmill compared to a cycle ergometer.

The data collected from these tests help to evaluate;

  1. Oxygen Uptake (VO2): This is calculated by assessing cellular oxygen demand and maximum oxygen transport,

  2. Respiratory Exchange Ratio: This is calculated by determining carbon dioxide output and oxygen uptake.

  3. Ventilatory Anaerobic Threshold: This helps to determine maximum exercise capacity by assessing the anaerobic metabolic threshold.

What Are the Contraindications of Cardiopulmonary Exercise Testing?

The contraindications for cardiopulmonary exercise testing are;

  1. Severe heart complications like active myocardial infarctions, heart failure, and cardiac arrhythmia (irregular heartbeat).

  2. Patients suffering from chronic obstructive pulmonary disorder (obstruction of the lung due to chronic inflammation) with severe asthma.

  3. Vascular disorders like aortic stenosis (abnormal functioning of the aortic valve) and deep vein thrombosis.

  4. Lung disorders like respiratory failure, pulmonary edema (presence of fluid in the lung), and pulmonary embolism (blockage of the lung vessels due to clothing).

Physical activity or regular exercise is good for health. However, understanding the physical potential is important. Also, such assessments are important for athletes and patients suffering from cardiopulmonary complications.
