Should I be worried about RBBB on my ECG report?

Hello doctor,

I had a recent ECG at an annual physical, which showed RBBB with no symptoms. I am an active 69-year-old female. The doctor placed a Zio patch for 24-hour monitoring as she heard a skipped beat on examination. Attaching my ECG for interpretation.

I can understand your concern.

Your ECG (electrocardiography) (attachment removed to hide the patient’s identity) that I have reviewed is showing the right bundle branch block or RBBB.

RBBB can be seen in various conditions. In asymptomatic and otherwise healthy individuals, it may be present at birth and does not indicate any significant health issue. If you do not have any health issues, enjoying a good and active life, then it is most probably a normal variant found in a minority of healthy individuals.

RBBB can also be due to conditions such as congenital heart disease and ischemic heart disease. Other conditions in which RBBB can be seen include lung issues.

Please remember that if you do not have any symptoms and your Zio patch recording was reassuring, then probably there is no pathology associated with RBBB.

The ECG you shared also showed RBBB, and it is otherwise negative for any other evidence of ischemia. Your blood pressure, LDL (low-density lipoprotein) or bad cholesterol, and HDL (high-density lipoprotein) or good cholesterol levels are also reassuring.

I would recommend a baseline laboratory test, including a complete blood count, serum creatinine, and electrolytes. I would also recommend a comprehensive echocardiogram and X-ray chest (PA view) just to rule out any structural heart issues or lung parenchymal disease. An echocardiogram may also indicate the presence of any ischemic pathology.

Thank you for your reply.

I have had two recent echocardiograms during the past three years, which were reviewed as normal. Also, complete blood count and metabolic panels are all within normal range.

It is very good to know that your echocardiogram was reported normal twice. This essentially rules out the presence of any significant structural heart issues. This is also reassuring to know that your laboratory tests were also within normal range.
