Hi doctor,

I had an abdominoplasty and developed a necrosis spot at the center of my incision, about half an inch wide, which the surgeon cut off. She also removed the dead tissue underneath. A week later, I went back, and she removed more dead tissue, a process she called debriding. The wound is now huge and deep, about a 2.5-inch wide hole. It looks like she cut out dead tissue almost down to my muscle, another 1.5 inches deep. I am prescribed Silverdine, I think it is called, and am instructed to clean and pack it twice a day. Is this a typical treatment? I felt like as soon as I changed my dressing, it was developing cream-colored slough inside again, which she just seems to cut away. I am a non-smoker. I underwent surgery seven months ago for a tummy tuck with liposuction. I am scared now. Please give your opinion.

The necrotic patch needs debridement (removal) so that it heals properly, even if it is deep (down to the muscle). The photograph you have attached (attachment removed to protect the patient’s identity) shows slough (a yellow, creamy substance, as you have mentioned) that also needs removal, either by medication or by debridement; this will be decided by your plastic surgeon. Once the wound is red and clean without any slough and develops good healthy tissues underneath, it will be closed by your plastic surgeon. Do not worry; it can be easily handled by a plastic surgeon. You just need to follow the surgeon's instructions.
