Hello doctor,
I am 18 years old. I have pimples and red marks on my face. If one gets off, a new one pops up. I have used Ayurveda, homeopathic face washes, and home remedies, but there are no results. I have been suffering from this issue for the past four to five years.
Please suggest something effective.

I have read your query and understand your concern.
Your problem is acne vulgaris. Acne vulgaris is a common skin concern in adolescents and may sometimes persist in adulthood. It is a skin disorder involving pilosebaceous follicles, or oil glands, and is therefore seen to include areas with dense oil glands. Face, chest, back, and upper arms are the common sites of involvement. In females, it may also be associated with menses irregularities or PCOS (polycystic ovarian syndrome). Do a USG (ultrasound) to rule it out.
You must follow certain dietary restrictions:
Avoiding oily and spicy food, junk food, milk products, and non-vegetarian food.
Drink lots of water, which hydrates your skin.
Avoid applying hair oil, because it can aggravate your forehead acne.
You should start with tablet Azithromycin 500 mg once daily for three consecutive days in a week for four weeks. Along with this, take capsule Isotretinoin 20 mg once daily and apply a topical antibiotic cream, like one percent Clindamycin gel, over the pimples on the face in the daytime.
Before starting the oral isotretinoin, the following tests must be done: liver function tests and lipid profiles. If these come back standard, you can start with the capsule Isotretinoin. However, you must know that Isotretinoin is a teratogenic drug (it can harm your fetus), which needs to be prescribed by a dermatologist. It requires regular observation and investigation.
There should be no pregnancy or conception for three to six months after stopping medication. Have a review after a month for further action.
