I am 29. Why did my acne return after stopping Isotretinoin?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been dealing with acne since high school. I used Oratane (Isotretinoin) for about seven months a few years ago, but once I stopped using it, the acne came back. I am 29 years old and weigh approximately 148 pounds.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

I am very sorry about your medical condition, and I am glad you are paying attention to changes in your body. But do not worry; I am here to take care of you.

Thank you for sharing that. Your acneiform eruption and exfoliating dermatitis may be linked to internal inflammation. Here is a breakdown of what might be happening and how to approach it beyond temporary fixes:

Why your acne keeps coming back:

  1. Hormonal imbalance: Especially if acne worsens around your period, on your jawline or chin, or during times of stress.

  2. Oily skin and clogged pores: If your skin is naturally oily, topical creams alone may not fully manage it.

  3. Diet or gut health: High sugar, dairy, or processed foods can trigger inflammation in sensitive individuals.

  4. Incorrect skincare: Using harsh products or over-washing can damage your skin barrier.

Long-term treatment options:

Medical options (stronger but effective):

  1. Oral medications: Tablet Doxycycline 100 mg by mouth thrice daily for two weeks.

  2. Hormonal treatments (such as birth control or Spironolactone for women), twice daily for one month.

  3. Topical Isotretinoin (Accutane), twice daily for one month.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
