Hello Doctor,

I have been experiencing pimples primarily on my forehead. I tried HydraFacials, but unfortunately, they did not help improve my condition. In response, I began using Tretinoin gel following the sandwich method. This is my third week of using it, and I am still experiencing whiteheads, which is concerning. In the first week, I used Tretinoin 0.025% percent and then increased to 0.5% percent on the second day. During the second week, I tried using 0.1% once, but it caused peeling on my forehead after three days, and then the peeling stopped. Now, in the third week, I have returned to using 0.025% Tretinoin, applying it only on Wednesday and Thursday nights.I am seeking guidance on how to better manage this treatment process and reduce the occurrence of whiteheads. Could you offer any recommendations or adjustments to my routine?

Based on your symptoms, I strongly believe you may have developed treatment failure due to your acneiform eruption. Have there been any changes to your medication history? I recommend discontinuing all current medications and starting the following treatment plan:

  1. Take Minocycline 500 milligrams orally three times a day for one week.

  2. Apply Topical Resorcinol 2 percent once daily, topical Tretinoin 0.1% twice daily, Azelaic Acid 20% once daily, or Dapsone gel 5 percent once daily for one month.
