Patient's Query

Hello doctor,

Thank you for your response. Here are my answers:

1. Since how long have you had the above mentioned problem sir? - From the last two months.

2. Do you have any difficulty in defecation? - I do not have any difficulty.

3. Is there any pain while defecating? - Yes, sometimes but not usually.

4. Do you have constipation? No.

5. Do you feel any external swelling or opening around the anal opening? I do not think so. Might be a small bump though.

6. Is there any pus-like discharge? No idea.

7. Have you done any tests so far? No.

8. If possible can you attach a picture for better assessment? (If you are comfortable) I have attached a photo of toilet paper that has been in the butt for a few hours.

Kindly suggest.

Thank you.

Hello,

Welcome back to icliniq.com.

Thank you for the answers. Based on your reply it is not hemorrhoid. It could be a simple anal fistula or localized perianal infection. In anal fistula, the patient usually complains of intermittent purulent discharge (which may be bloody) and discomfort which increases until temporary relief occurs when the pus discharges. Do you find any similarity in the symptom presentation written above? Because in the tissue paper, it appears to be mucus mixed with pus (attachment removed to protect patient's identity). Is it smelly? Now to distinguish the above two differential diagnoses you need a physician visit to examine the perianal area and to do a proctoscopy to look for any internal or external opening. I can guide you if you could attach a picture of the perianal area to look for skin infection or opening if present (if you are comfortable). Please reply to the above questions as well to guide your diagnosis and further workup needed to confirm it and medications if any. I hope this helps you.

Thank you.
