What causes swelling above anal opening?

I have a swelling in the genital area just above anal opening. I do not have pain, discharge, or tenderness. Kindly help.

It looks like chronic anal fissure, skin tag or anal tag. Due to prolonged history, it becomes painless. It is an anal fissure, which is a longitudinal ulcer in the anoderm of the distal anal canal. This condition is usually associated with hypertonia of the internal anal sphincter, which increases the traumatic effect of passing hard stools. Here is the recommended approach:

Consume a high-fiber diet to ease bowel movements.

Take syrup Duphalac, two teaspoons, three times daily.

Drink plenty of water throughout the day.

Sit in warm water for around 15 minutes twice daily.

Apply Lignocaine gel mixed with 0.2% GTN (Glyceryl Trinitrate ointment)ointment two to three times daily.

Take a tablet of Naproxen sodium, one tablet twice daily.
This regimen should be continued for at least two weeks to see noticeable improvement. In most cases, these conservative measures will resolve the issue. If symptoms persist, surgery may be necessary, with a procedure called lateral anal sphincterotomy being the typical choice. But in your case, it does not look like an acute case as you are not experiencing any history of pain, itching, or discharge.
