My brother has hereditary angioedema. How does Icatibant work?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My 28-year-old brother has hereditary angioedema, and he has had multiple swelling episodes affecting his face and throat. The last one required an ER visit. We were told about preventive treatments and on-demand medications like Icatibant.

How quickly do these work in emergencies, and should he keep an injector at home at all times?

Please suggest.

Hello,

Welcome to icliniq.com.

I understand your concern.

I hope you are doing well, and you have to know that hereditary angioedema is a condition where swelling episodes can develop unpredictably and, as you have seen, can become life-threatening when the throat is involved, so having a clear emergency plan is essential.

The on-demand treatments, such as Icatibant, are specifically designed for this situation.

When injected under the skin at the first sign of an attack, Icatibant typically begins to relieve symptoms within about 30 to 60 minutes, and many patients experience significant improvement within a few hours.

The earlier it is given, the better it works and the more likely it is to prevent the progression of the swelling.

Because attacks can happen suddenly and access to the emergency room may take time, current recommendations are that patients with hereditary angioedema should keep at least one, and ideally more than one, dose of their on-demand medication with them at all times and be trained to self-administer it.

This is especially important for anyone who has had laryngeal involvement in the past. Even when treated at home, any throat symptoms still require urgent medical evaluation, but early treatment can be life-saving and may prevent the need for airway intervention.

In addition to on-demand therapy, many patients who have frequent or severe attacks benefit from long-term preventive treatment, which can significantly reduce how often attacks occur.

Your brother’s specialist can determine whether he meets the criteria for prophylaxis based on the frequency and severity of his episodes.

So yes, Icatibant works quickly when used early, and it is strongly recommended that your brother carry his medication at all times and know exactly when and how to use it, along with having an emergency plan for any airway symptoms.

I hope this helps.

Thank you.
