Hello doctor,

I have been experiencing severe episodes of nervousness, anxiety, and physical symptoms for the past two months. These episodes started after work-related stress and a layoff. During these episodes, I feel like I am choking, my blood pressure spikes, I struggle to breathe, and I feel like I might lose my life. I have done comprehensive medical tests, including a cardiac check-up, CT scan of arteries, and lung function tests, and all results came back normal. Despite this, the episodes persist, and they are severely affecting my daily life. I have tried relaxation and breathing techniques, but they have not helped. I would like advice. Currently, I take medications like Exforge 5 mg /80 mg and Nebilet 5 mg.

I understand your concern.

As per your mentioned symptoms and history, I think you are going through anxiety. All tests are normal, which suggests anxiety is secondary to work-related stress. Please answer the following:

1. For how long do such symptoms last?

2. Do you have any drinking and smoking habits?

I suggest you start anti-anxiety medicine. Start Paroxetine with Clonazepam at night and Etizolam and Propranol combination in the morning and afternoon. Etilaam Pro 20 (Etizolam 0.5 mg and Propranolol 20 mg) will help to control immediately, and Paroxetine will help in the long term. Start physical exercise daily.

Thank you for your response.

I do not smoke or drink. The duration of my symptoms depends on the situation. Sometimes, they last for an hour or so, but other times, they persist for longer. On certain occasions, I have had to visit the emergency room due to extreme blood pressure, and after stabilizing me, the doctors have mentioned they did not give any medication apart from something like Paracetamol through an IV.

Recently, I have noticed that when I wake up in the morning, I feel a heaviness in my chest, and my blood pressure spikes. I am wondering if my nasal septum deviation (on the left side) could be causing breathing issues while I sleep, which triggers this, although I have never experienced anything like this before. Could this be related?

I understand your concern.

As per your mentioned things, it is anxiety only. DNS (deviated nasal septum) is not the reason for this problem. All symptoms are of anxiety only and are not related to DNS. Start anti-anxiety medicine.
