Patient's Query
Hello doctor,
I am a 39-year-old female with uncontrolled asthma. My Budesonide inhaler is no longer effective. I am wheezing constantly and struggling to sleep. Can I take a steroid like Betamethasone, or is there another option?
I am worried about having an attack at work. I have heard about biologics. Would they help? What about using a nebulizer? I am open to trying anything at this point. Please help.
Thank you in advance.
Hello,
Welcome to icliniq.com.
I read your query and can understand your concern.
Based on your history, it appears that you may be suffering from bronchial asthma (a chronic respiratory condition that affects the airways in the lungs).
The key to managing asthma is the regular use of inhaled medications. To control asthma effectively, it is important to consistently use the right inhalers at the correct dose, using the proper technique.
Investigations for newly diagnosed or uncontrolled bronchial asthma:
Complete blood count.
Serum total IgE (immunoglobulin E).
Chest X-ray.
Pulmonary function test (spirometry pre- and post-bronchodilator).
Possible causes for repeated exacerbations:
Presence of comorbid conditions such as allergic rhinitis, GERD (gastrointestinal reflux disease), or obesity (excess body fat).
Incorrect inhaler technique or inconsistent use of the inhaler.
Use of incorrect inhaled medications.
Advice if bronchial asthma is diagnosed:
Inhalers: Start using inhalers regularly. For example, MDI or metered dose inhaler Budetrol (200/6), two puffs twice daily, with a spacer, followed by a mouthwash. Inhalers should be used daily for at least six months without skipping doses unless advised by a pulmonologist.
Spacer: Use a spacer with the inhaler. The proper technique can be learned through online resources like videos.
Consultation: Regular consultations with a pulmonologist, either in person or virtually, are recommended (at least once a month).
Vaccinations: Ensure that you are up to date with vaccinations appropriate for your age.
Control comorbid conditions: Manage any other health conditions you may have.
Please get the recommended investigations done and follow up with the results in one week. We will evaluate whether your asthma is under control and discuss further treatment options, such as biological medications, if necessary.
I hope this helps.
Kindly revert so I can assist you further.
Thank you.
