How to manage severe atopic dermatitis in a woman?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am a 28-year-old woman who has been struggling with severe atopic dermatitis since childhood, but it has significantly worsened over the past year.

My eczema flares are horrible around my menstrual cycle, and I have noticed they get worse during ovulation and right before my period starts.

The itching is so intense that it disrupts my sleep, and I am concerned about how this might affect my fertility plans since my husband and I are trying to conceive.

I also have seasonal allergies, which seem to make everything worse. The constant scratching has left dark patches on my skin, especially on my inner thighs and around my breasts, which makes me feel self-conscious about intimacy with my partner.

I am currently using topical steroids, but I am worried about long-term use, especially if I get pregnant. Additionally, I have been experiencing irregular periods for the past few months; sometimes they are 35 days apart.

  1. Is it normal for eczema to worsen during certain phases of my menstrual cycle, and could hormonal changes be triggering my flares?

  2. Are the topical steroids I am using safe if I become pregnant, and what alternative treatments would you recommend during pregnancy?

  3. Could my chronic eczema and the stress it causes be affecting my fertility or causing my irregular periods?

  4. The dark patches (hyperpigmentation) on my intimate areas are affecting my confidence - are there safe treatments to address this?

  5. How can I manage my eczema flares during allergy season without compromising my reproductive health?

  6. Is there a connection between my seasonal allergies and the worsening of both my eczema and menstrual irregularities?

  7. What lifestyle modifications should I consider to improve my skin condition and menstrual health while trying to conceive?

Please guide.

Hi,

Welcome to icliniq.com.

I read your query and understand your concerns.

Hormonal changes during ovulation and in the premenstrual phase can commonly trigger eczema flares. This is not unusual and is often seen in individuals with hormonally sensitive skin.

Regarding topical steroids, mild to moderate options such as hydrocortisone or desonide are generally considered safe during pregnancy. However, high-potency steroids or long-term use should be avoided unless absolutely necessary and only under medical supervision.

Safer alternatives during pregnancy include consistent use of moisturizers or emollients at least three to four times daily. Topical calcineurin inhibitors like Tacrolimus or Pimecrolimus may also be used under a doctor’s guidance.

Chronic eczema and the associated stress can disrupt sleep and impact hormonal balance, which may contribute to irregular menstrual cycles. While eczema itself does not directly affect fertility, it is important to continue monitoring your cycles and seek a gynecological evaluation if irregularities persist.

Post-inflammatory hyperpigmentation in intimate areas is a common result of inflammation and scratching. The safest approach during pregnancy is to focus on gentle skincare, regular moisturization, and sun protection for exposed areas, rather than using strong lightening agents. Once the underlying inflammation is controlled, pigmentation often improves over time.

Seasonal allergies can worsen eczema symptoms, and using a non-sedating antihistamine such as Cetirizine or Loratadine once daily may help. These are typically safe in pregnancy but should be confirmed with your healthcare provider.

It is important to maintain a balanced lifestyle for overall skin and reproductive health. I suggest the following tips for you.

  1. Maintain regular and adequate sleep.

  2. Practice stress-reduction techniques such as yoga, meditation, or light exercise.

  3. Follow a nutrient-rich diet, including omega-3 fatty acids, vitamins, and protein.

  4. Avoid harsh soaps and prolonged hot showers.

  5. Refrain from scratching to prevent worsening of skin lesions.

  6. Keep nails trimmed short to reduce skin trauma from itching.

  7. Track your menstrual cycle and eczema symptoms in a diary to help identify patterns and triggers.

I hope this answers your query. Feel free to reach out anytime.

Thank you.
