Why do I have itching, dryness, and skin discoloration?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been noticing itching, dryness, discoloration after scratching, which later turns into brown scars and small, rough patches on the skin. The affected areas sometimes feel itchy, not only on the forearms but also on the thighs and back. These symptoms have been present for some time and do not seem to be improving on their own. I would like to understand the possible cause and receive advice regarding treatment and skin care.

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand how uncomfortable and frustrating it can be to deal with persistent itching, dryness, discoloration after scratching, and rough patches on the skin, especially when the symptoms involve multiple areas of the body.

Based on the description, the condition may be suggestive of atopic dermatitis or lichen simplex chronicus, where repeated itching and scratching lead to thickened skin and brown discoloration known as post-inflammatory hyperpigmentation (PIH).

To guide treatment more accurately, a few additional details would be helpful, such as the exact duration since the symptoms started and whether they are gradually worsening. It is also important to know whether the itching is worse at night, whether there is any personal or family history of eczema, asthma, or allergies, and whether any new soaps, detergents, perfumes, creams, or clothing fabrics were introduced before the problem began.

Please also mention if there is any scaling, redness, or oozing from the affected patches, and whether there are any associated medical conditions such as diabetes, thyroid disorders, anemia, or recent unexplained weight loss. Information about any current or past treatments already tried and the response to them would also be important.

In the meantime, general skin care measures can help manage the symptoms. Use a mild, fragrance-free cleanser once daily and avoid harsh soaps and loofahs. A gentle syndet or plain moisturizing soap can be used. Moisturize generously at least twice daily with a thick, bland moisturizer such as ceramide-based cream or petroleum jelly, especially immediately after bathing. Avoid scratching as much as possible, keep the nails short, and wear loose, breathable cotton clothing.

For itching, Cetirizine hydrochloride 10 milligrams (mg) can be taken once daily at bedtime for one week, unless contraindicated. Once the itching is under control, guidance can be provided for treating the pigmentation safely.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
