How can I manage my atopic eczema symptoms?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I have been experiencing itching on my left hand for many years, and there seems to be no way I can get rid of it. I took some pictures under a magnifying glass, so if you could provide a diagnosis, I would greatly appreciate it.

The current medication details include Euthyrox 100 mg for thyroid.

Kindly suggest.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

I am here to give you the best care possible.

It appears to be atopic eczema based on the attached image (attachment removed to protect the patient’s identity). Apply Fucibet (a combination of Betamethasone and Fusidic acid) cream at night for seven days (on the affected area).

Use Moisturex (liquid paraffin and white soft paraffin) soft lotion or Dermocalm (calamine and liquid paraffin) lotion three to four times daily for ten days. If there is irritation, take one tablet of Teczine-M (a combination of Levocetirizine and Montelukast) at night after food for seven days.

Here are my suggestions to ease your discomfort:

  1. Avoid scratching or rubbing the lesions.
  2. Avoid injury, trauma, occlusion, and friction at the site.
  3. Avoid known triggers.
  4. Avoid contact with harsh soaps and detergents.
  5. Use a mild soap for bathing.
  6. Avoid prolonged contact with water.
  7. Avoid frequent handwashing.

Follow up after ten days.

I hope this has helped you.

Please feel free to reach out to me again in case of further queries.

Thank you.

Patient's Query

Hello doctor,

Thank you for your help.

I performed the therapy for tinea around the anus as you said. It helped me a little. I am sending you new pictures, taken with a magnifying glass, so that you can identify what it is. It itched terribly last night.

Kindly review and suggest.

Thank you.

Hello,

Welcome back to icliniq.com.

I do understand your concern.

I have read your query and reviewed the pictures (attachments removed to protect the patient's identity). It appears to be an infected lesion (superficial fungal infection with secondary bacterial infection). I suggest you follow the suggestions below:

  1. Apply Candid (Clotrimazole) cream twice daily for two weeks.
  2. Take Atarax (Hydroxyzine) 10 mg twice daily after meals for seven days (if you have itching).
  3. Do not scratch or rub the lesions.
  4. Wear loose, comfortable thin cotton clothes or inners.
  5. Maintain proper hygiene.
  6. Keep a check on sugar levels.
  7. Clean and dry the area properly.

I hope this helps you.

Regards.
