Hi doctor,

I am struggling with severe obesity (BMI 45) and advanced osteoarthritis in both knees. My orthopedic surgeon refuses knee replacement until I lose significant weight, while my bariatric surgeon recommends a gastric sleeve first, followed by knee replacements after losing 80 to 100 pounds.

How do I exercise post-bariatric surgery with such painful knees? Also, how can I manage post-operative pain from bariatric surgery while taking arthritis medications, especially since NSAIDs are restricted?

What is the best sequence to minimize complications? How long should I wait between surgeries? What are my pain management options after bariatric surgery? I feel trapped between two urgent medical needs with no clear way forward.
