Can scrub typhus cause black spots on the feet?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My uncle, who is 55 years old, has been very sick for the last few days with a high fever, chills, and severe headaches. He also has a strange black spot near his ankle that resembles an insect bite. The local doctor suspects scrub typhus but mentioned they do not have the proper tests available. He is extremely weak and unable to get out of bed. We do not know how he got this, but he was working in the fields in a village last week. The doctor prescribed antibiotics, but how long will it take for them to work? Is this condition contagious? Also, should we consider any other treatment or steps to prevent his condition from worsening?

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern about the unavailability of specific diagnostic tests at the moment. However, conducting basic blood tests like a complete blood count (CBC) can provide valuable insights and should be prioritized if feasible. Scrub typhus remains a possible diagnosis, and the prescribed antibiotics should help alleviate symptoms within a couple of days. Additionally, anti-inflammatory medications may be used as supportive treatment for body aches and pains. If you can share the list of medications already prescribed, I can suggest any additional treatments if necessary.

In some cases, scrub typhus may resolve on its own, but for individuals with weakened immunity especially those in your uncle's age group, it can become severe or even fatal. It is important to monitor his vital parameters at home, such as blood pressure and body temperature, and watch for any signs of disorientation or altered mental status. These could indicate a worsening condition and would require immediate medical attention.

I hope this helps you.

Thank you.
