Hi doctor,

I am a 35-year-old male, and I did a kidney function test, and the creatinine level came out to be 1.8 mg/dL, and the urea 14 mg/dL. There are no symptoms except mild, occasional dizziness.

I have been having fluctuating blood pressure for a period of seven months, which fluctuates between 115/77 mm Hg and 130/95 mm Hg. I only take Amlodipine when I notice it is high. I do not drink or smoke. Please help.

Your ultrasound abdomen being normal is a very positive finding. It means the kidneys are of normal size and structure, with no stones, obstruction, or chronic damage.

To summarise your reports:

Your blood pressure (BP) readings are borderline, and taking Amlodipine only when BP rises can cause fluctuations. Blood pressure medications are meant to be taken regularly, not intermittently, as unstable blood pressure can affect kidney function over time.

1. Maintain adequate hydration.

2. Repeat serum creatinine and urine routine in seven to 10 days (first-morning urine preferred).

3. Do a urine protein–creatinine ratio.

4. Monitor BP twice daily and avoid self-adjusting medication.

5. Avoid painkillers and protein supplements.

If repeat tests return to normal, no further action is required. If creatinine remains elevated or protein persists, further evaluation will be done.
