Hi doctor,
I am a 38-year-old woman with borderline personality disorder, generalized anxiety disorder, ADHD (inattentive type), restless leg syndrome, and insomnia. I have been on Venlafaxine hydrochloride150 mg for anxiety and low mood, Aripiprazole 2.5 mg for anxiety (particularly rumination and intrusive thoughts), and Lisdexamfetamine dimesylate 60 mg for my ADHD for many years. However, I still often feel anxious, and I wake up at midnight a few times a week, unable to get back to sleep. I struggled with this sleep issue before adding Lisdexamfetamine dimesylate to my medication regime. I drink coffee once in the morning when I wake up. I am happy with Aripiprazole and Vyvanse. Aripiprazole is the only thing that keeps my restless leg syndrome at bay, strangely but I do not know that Venlafaxine hydrochloride helps me. What are some other antidepressants I could try that might help my anxiety and insomnia? I have been on numerous SSRIs and SNRIs already. Please guide me.

I can understand your concern.
As per your mentioned symptoms and history, I think you require SSRIs (selective serotonin reuptake inhibitors) like Paroxetine. Start with Paroxetine 12.5 mg and then increase it to 25 mg after consulting your physician. Start physical exercise daily. Avoid smoking and drinking if any. Start psychological sessions particularly CBT (cognitive behavioral therapy).
