I have HR-positive BC. What are the treatment options?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have just been diagnosed with hormone receptor-positive breast cancer at the age of 55.

  1. What treatment options are available to me?
  2. How will my age and being post-menopausal impact my treatment plan?
  3. The tumor is small and has not spread to my lymph nodes. Does this affect the approach?
  4. I have heard about genomic tests like Oncotype DX. Should I consider having one?
  5. Are there any new treatments or clinical trials that I should be aware of?
  6. I am concerned about the side effects of hormone therapy, particularly regarding my bones and heart. How can these side effects be managed?
  7. What can I expect in terms of recovery and long-term outlook?

Kindly help.

Thank you.

Hello

Welcome to icliniq.com.

Thanks for your query.

Hormone receptor-positive breast cancer means that your cancer cells grow in response to estrogen or progesterone. This type of breast cancer generally has a good prognosis when detected early and treated appropriately.

Since your tumor is small and has not spread to the lymph nodes, breast-conserving surgery (lumpectomy) followed by radiation therapy is a common approach. A mastectomy might be recommended depending on the tumor's characteristics.

If you choose a lumpectomy, radiation therapy is recommended afterward to reduce the risk of recurrence in the remaining breast tissue.

Hormone therapy is crucial for hormone receptor-positive breast cancer. It helps reduce the risk of cancer returning by blocking estrogen’s effects or lowering estrogen levels in the body.

As you are post-menopausal, aromatase inhibitors are preferred over tamoxifen, which is commonly used for pre-menopausal women. Aromatase inhibitors reduce estrogen production and are effective in post-menopausal women.

Given that your tumor is small and has not spread to the lymph nodes, chemotherapy may not be necessary. The Oncotype DX test analyzes the expression of certain genes in your tumor to predict the likelihood of recurrence and determine whether you would benefit from chemotherapy. Since your cancer is hormone receptor-positive, small, and has not spread to the lymph nodes, this test can provide valuable information. Your age and post-menopausal status are important factors in treatment planning.

Aromatase inhibitors can lead to bone thinning or osteoporosis, increasing the risk of fractures. To address this do the following:

  1. Monitor your bone health with bone density tests.

  2. Take calcium and vitamin D supplements.

  3. Engage in weight-bearing exercises to strengthen your bones.

  4. Regular monitoring of your heart, maintaining a healthy lifestyle, a balanced diet, regular exercise, and avoiding smoking can help reduce risks. Targeted therapies, such as CDK4/6 inhibitors, are new treatments that may be available.

  5. Most women with early-stage hormone receptor-positive breast cancer live long, healthy lives after treatment. Regular follow-ups with your oncologist are essential.

I hope this guidance helps you. Please do not hesitate to reach out if you need further assistance.

Thank you.
