This is a complex situation that needs your guidance. I am 61 years old with stage 2B breast cancer and severe coronary artery disease (three stents placed last year, ejection fraction 40 percent). I started Herceptin (Trastuzumab) last month but have been experiencing chest pain and severe shortness of breath. My blood pressure fluctuates between 180/100 and 90/60.
I am currently taking Metoprolol, Lisinopril, and Aspirin. A recent echocardiogram showed a further reduction in ejection fraction to 35 percent. Can I safely continue Herceptin with a low ejection fraction? What cardiac monitoring do I need? How should blood pressure fluctuations be managed during treatment?
