Hello doctor,

I have an infection; my whole scrotum is red, with brown spots lining into a circle around my penis, and half of my penis shaft is red up to the head and has red spots. The redness looks like a bunch of tiny red line patterns. Every time I masturbate, the redness spreads down my penis across the brown circle.

What do you think this can be?

Redness or irritation in the groin and scrotal area can happen for a few different reasons. Sometimes it is irritant or contact dermatitis, which basically means your skin is reacting to things like rubbing, sweat, soaps, lubricants, condoms, or latex. When that happens, the skin can look red or streaky or even have tiny lightning-like lines; it is just your skin signaling irritation.

Other times, it could be a fungal infection, like tinea cruris or candidal intertrigo. This usually shows up as redness around the scrotum or groin, sometimes with a ring-shaped border. Heat, sweat, and tight clothing can make it worse. A common mistake is using steroid creams like desonide on it; these can reduce redness temporarily, but may actually make a fungal infection persist or spread, which is called tinea incognito.

It could also be eczema or lichen simplex, which happens when the skin is rubbed or scratched repeatedly, leading to thick, itchy patches. And in some cases, redness could be from other inflammatory skin conditions, like psoriasis, which can cause inflammation, scaling, or thickened skin.

Sexually transmitted infection (STI)-related rashes are less likely given the long duration and the flares with a friction pattern, but still worth ruling out depending on sexual history. What I would advise you to do now

  1. Stop all potential irritants for two weeks. No scented soaps or body wash on genitals (use plain water or a gentle non-soap cleanser).

  2. Wear loose cotton underwear, keep the area dry, and change after sweating. Avoid shaving or trimming.

  3. Do not mix steroid creams with a possible fungal rash.

Since you have already used Clotrimazole at different times and this is lingering, I would avoid steroid creams on the penis or scrotum until we are sure there is no fungus (steroids can worsen fungus).

  5. Any sexual activity in the last year, and were you ever tested for chlamydia, gonorrhea, syphilis, or HIV (human immunodeficiency virus)?

There have been times when it used to burn while peeing, but other than that not been itching or burning. It does feel a little like scales around the circle. Yes, it is darker around the penis and scrotum area. No, just the same products I usually use. No, I have never had sex, just masturbation.

This can include things like intertrigo (a rash in skin folds), irritation from contact with soaps or clothes, such as in contact dermatitis, friction, and even thickened skin from repeated rubbing or scratching.

These types of skin conditions often get worse with heat and sweating. Barrier creams can help, and sometimes a short course of steroid cream can calm things down. The lightning pattern you noticed is probably just a sign of irritated blood vessels or inflamed skin; it is not dangerous.

To reduce irritation, you may

  1. Try to avoid masturbation for two to three weeks if possible. When you do resume, use plenty of lubrication.
  2. Switch to loose, cotton underwear and keep the area dry. Change out of sweaty clothes as soon as possible.

To repair the skin barrier, use plain petrolatum (like Vaseline) or zinc oxide in a thin layer twice a day, especially at night.
