What could cause a burning sensation in the penis after sex?

Hi doctor,

Roughly six days ago, I noticed a sudden onset of persistent burning in my penis. It does not really burn when I urinate, and if anything, it soothes the burning when I urinate. It is a persistent mild burning. I have been sexually active with one girl for the last eight months, as we are both monogamous with each other. Neither of us has an STI. I have also been very sexually active with this same person. Nothing I do makes the burning worse or better. Thinking this can be some sort of inflammation, I have remained abstinent (sex and masturbation) the last few days, but this has not helped the burning. I have to add that this exact issue happened to me, with the same symptoms four years ago. I did all of the STI tests, bacteria tests, etc, and all of it returned normal. Ultimately, the issue went away on its own without me doing anything. The sudden onset of burning six days ago was after a day of sexual intercourse with my girlfriend. Roughly four hours after sex, the sudden onset of persistent burning started.

Hi doctor,

Thanks for the reply.

I am not diabetic or hypertensive. I had no prior surgeries. I do sit for long periods. There is no difficulty at all with urine flow. No symptoms either like cloudy or smelly urine. The only symptom is the persistent dull burning that started a few hours after sexual intercourse a week ago.

I understand your concern.

Based on your response I believe you may have a component of urethritis and prostatitis. It would be wise to get some tests done, namely:

  1. Uroflowmetry.

  2. USG (ultrasonography) KUB - Ultrasound of the kidneys, ureters, and bladder.

  3. Semen for culture and sensitivity.

You may also benefit from the following medications (depending on your country of residence you may need a prescription from your local practitioner):

  1. Tablet Alfuzosin 10 mg once at night for 14 days.

  2. Tablet Trypsin and Chymotrypsin once in the morning, noon, and night for 14 days.

  3. Tablet Cranberry extract and D-mannose once in the morning and at night for 14 days.

As well as a probiotic tablet twice a day for 14 days. Please avoid sitting for prolonged hours at a stretch - give your bottom a break after every one to one and a half hours hours. Drink at least three liters of water a day.

Thanks again.

How do you think this happened? Urethritis makes sense due to sexual intercourse but how does prostatitis occur? Also, do you think this will go away on its own or do I need medicine? The last time this happened it went away on its own after six months.

Prostatitis is very common amongst men aged 18 to 40. Especially amongst men who sit for prolonged periods at a stretch, smokers, men who do not ejaculate frequently, and men who have bad bacteria in their gut.
