Patient's Query
Hello, Doctor,
My uncle is 68 years old. He recently tested positive for Candida auris in a hospital swab. He had a recent UTI and fever, and now they are keeping him isolated. He also has a history of psoriasis but has not been on immunosuppressants for years. I am reading online that Candida auris is super resistant. He is already frail, and we do not want this to turn into something deadly. Please tell me,
Should we be worried?
Should we push for antifungal susceptibility testing?
Kindly help.
Hello,
Welcome to icliniq.com.
I read your query and can understand your concern.
Candida auris (C. auris) is a multidrug-resistant fungus that often colonizes the skin, wounds, or urinary tract and can lead to invasive infections like bloodstream infections or sepsis. It can be serious, even life-threatening infections, especially in vulnerable individuals who have a history of medical illnesses, including diabetes, hypertension, or immunocompromised individuals. As it spreads with contact, isolation with gowns and gloves is crucial.
As you have mentioned, the history of psoriasis, but not on any immunosuppressants, the blood levels of anti-inflammatory markers need to be measured. This, along with a recent urinary tract infection (UTI) episode, can be a high-risk factor for Candida auris in the patient.
I advise your uncle the following things,
Check for drug sensitivity, as C. auris is a multidrug-resistant fungus; drug sensitivity and specificity need to be determined by culture.
Improvement in the symptoms will ensure adequate antifungal drug.
Liposomal Amphotericin B and fluconazole can be given as antifungal drugs. Also, combination antifungal therapy can be used.
Investigations needed for him are as follows,
Complete blood count.
Erythrocyte sedimentation rate (ESR).
Kidney function test.
Liver function test.
Blood.
Urine and site culture and sensitivity.
I hope this information helps you.

Thank you.
