Can botulinum injections help cervical dystonia spasms?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 46 years old and just got diagnosed with cervical dystonia. My brain MRI is normal, but my EMG from a neurologist showed abnormal contractions of neck muscles. So, I have been on Clonazepam and physiotherapy, but it still looks quite prominent.

I wanted to ask whether injections of botulinum toxin would do anything to alleviate the tightness in the muscles and how long the effect of the injections usually lasts. Are there long-term side effects with repeated injections?

Moreover, can stress or lack of sleep advance the spasms? My neurologist also mentioned "sensory tricks"; could you explain what those are and whether they help meaningfully with symptom relief?

Please help.

Thank you for your guidance.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

Cervical dystonia occurs due to an involuntary contraction of the neck muscles, causing the neck to twist or tilt. It is an ongoing issue, but very controllable, and the fact that you are taking proactive measures with physio and medication already puts you ahead.

1. Botulinum toxin injections are the most successful remedy for cervical dystonia. Their function is based on briefly relieving contracting muscles by obstructing certain signals sent via nerves.

Onset of Effect: It usually becomes effective within five to 10 days.

Duration: The duration for benefits will be three to four months.

  • Repeat injections: These are done at three- to four-month intervals.

  • Long-term safety: Botulinum toxin is very safe even on prolonged administration. Side effects, if any, like mild weakness, soreness, and transient difficulties in swallowing, are usually transient. The chances of resistance due to antibody production are very low if there is an ample gap between doses.

2. Triggers include stress, anxiety, and poor sleep, which can intensify spasms by increasing overall muscle tension and nervous system sensitivity. Good sleep habits, stress-management strategies, and relaxation practices such as yoga or mindfulness can help reduce symptom flares.

3. Sensory tricks (geste antagoniste) are simple touches or movements, such as gently placing a hand on the chin, cheek, or back of the head, that can momentarily ease abnormal neck postures. They work by providing different sensory input that interrupts the involuntary muscle signals. Many patients find these techniques surprisingly helpful, even if the relief is temporary.

In summary, botulinum toxin injections are a safe and highly effective option, especially when combined with ongoing physiotherapy, adequate rest, and stress-reduction strategies. With consistent care, most individuals experience meaningful improvements in comfort, posture, and day-to-day function.

I hope the information helps.

Kindly revert so I can assist you further.

Thank you.
