Hello doctor,

My wife, who is 27 years old, has been feeling a pricking pain in her chest for the past 15 to 20 days. Also, she feels pressure across the middle of her chest sometimes. I consulted a physician, and she provided me with 15 days of medicine. However, I also got her ECG test done, and the reports indicated that it was normal. During the medication, the condition was better. Only one or two times did she experience pain or pressure. Her medicines were stopped for three days, and this morning, she again felt/a little pressure. Uneasiness during long breaths. Please advise what to do. What tests should be done to diagnose the problem?

I read your query and understand your concern. Most probably acidity, gastritis, and reflux disease. Does she also have upper abdominal pain, nausea, bloating, burping, increase in pain on food, sour water feeling in the throat, or chest burning? These are the other symptoms and may be associated. The probability of heart disease is less, considering age, symptoms are not very compatible, ECG (echocardiogram) is normal, and relief with antacids. Heart symptoms typically come with exertion and relief with rest. Anyway, there is no harm in getting her evaluated; she will need an echocardiography and treadmill test, which will reliably rule out the disease.

She should avoid fatty, oily, and spicy diets. Have some walks after eating food instead of resting. Have multiple small meals instead of heavy meals. Have regular sleep habits and avoid stress. Lots of green leafy vegetables, fruits. Elevate the head end of the bed or use pillows during rest and sleep. She should get a prescribed tablet of Pan (Pantoprazole) DSR 40 mg or Esomeprazole Domperidone combination before breakfast once a day for two weeks. If not adequately relieved, then you should add syrup Sucral (Sucralfate) O two teaspoons three times a day for a week.
