Hi doctor,

My father, aged 73, already has chronic heart disease and has recently taken blood and urine tests. The result is as follows.

  1. Urea- 63.0 mg/dL.
  2. Creatinine - 1.8 mg/dL.
  3. HbA1C - 6.6 %.
  4. Potassium (K) - 3.74 mmol/L.
  5. Chloride - 109.0 MEq/L.
  6. Cholesterol and HDL ratio - 3.3 %

What are the food restrictions he has to follow strictly, what can be done to improve the above readings, and what medications for the above readings? Currently, he is taking tablets for heart ailments and diabetes as well. Please help.

I understand your concern.

Please help me with some details, does he have any difficulty passing urine? Like straining to pass urine, weak flow, feeling of incomplete bladder emptying? Does he have to wake up multiple times at night to pass urine? Has an ultrasound been done for him in the recent past? From the values you have mentioned above, it is evident that diabetes is not optimally controlled. HbA1C (glycated hemoglobin) should be less than six with good control. His creatinine and urea increase signifies that he has some renal damage which may be age-related along with contributing effects from diabetes and hypertension. I suggest, he should be on a low salt, low sugar diet, avoiding things like rice, potato, banana, chocolate, and nuts. He can have the mentioned diet but in very moderate amounts. As a renal protective agent, you can start him on tablet Nephrosave (Taurine and Acetylcysteine), one tablet once a day (maybe available as a different brand in your location). He should also see a nephrologist for a consultation.

Hi doctor,

Thank you very much for your quick reply, doctor.

I have provided below the responses. He has no difficulty passing urine. During midnight two to three times maximum wakes up for passing urine. But sometimes he feels hungry, so he is forced to take biscuits and a half cup of warm water. The quantity of urine is about 100 to 150 mL. He has had no ultrasound done for him in the recent past. Please suggest any fruit or something else for intake during midnight or early morning instead of biscuits. Also, the heart doctor per day advised taking 750 mL of water.

I understand your concern.

First, I would suggest you get an ultrasound KUB (kidney, ureter, and bladder) and a uroflowmetry test done for him as soon as possible. This will rule out any bladder outlet obstruction and give a baseline idea about his kidneys. As far as a midnight snack is concerned, low sugared biscuits are pretty good for diabetes since they are low in sugar. He could also try papaya, watermelon, half an apple, and half a banana.

How much is water intake required for my father per day? Today his sugar level post-food is 121 mg/dl in the morning.

The sugar level seems to be fine. As far as water intake is concerned, if he has a heart condition and his cardiologist has advised water restriction to 1 liter per day should be enough. I would suggest you check with your cardiologist as well.
