Hello doctor,

My libido has been low for about three years. It began when I was regularly using recreational drugs, including cocaine and MDMA, often combined with sex. Since then, I have had a hard time becoming sexually aroused without medications. The rare exceptions are very stimulating situations, like explicit or kinky talks.

Repeated use of MDMA and cocaine in sexual settings can, in some people, condition the brain to associate sexual arousal with intense stimulation, novelty, or drug use. This does not necessarily mean permanent damage, but it can make ordinary sexual cues feel much less exciting for a period of time. Testosterone levels are normal, which is reassuring.

Many people improve over time, especially if they remain abstinent from recreational drugs. Recovery often involves reducing reliance on highly stimulating sexual content, including extreme pornography or kinky chatting, if it has become the main source of arousal, allowing the brain's reward system to readjust, maintaining regular exercise, good sleep, stress management, and rebuilding sexual experiences in normal, non-drug contexts.

Improvement can take months, particularly after repeated stimulant use. Are you completely abstinent from cocaine and MDMA now? Do you still get morning erections?
