What is the best colorectal cancer treatment after surgery?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

My mom had surgery to remove colorectal polyps recently. Her pathology results show colorectal cancer grade IIA (T3 N0 Mx). What are the best treatment options after surgery for her?

Please help.

Hi,

Welcome to icliniq.com.

I understand your concern.

It is stage II cancer, and it is curable. She should go for eight cycles of Folfox-based chemotherapy. At the age of 67, a full dose can have serious side effects. She should receive a 50% dose in cycle 1. If she tolerates it well, then a 75% dose can be given in the remaining seven cycles. Otherwise, continue with a 50% dose. I hope this helps.

Patient's Query

Hi doctor,

What is Folfox-based chemotherapy? Also, what are 50% and 70% dosages in terms of milligrams?

Please help.

Hi,

Welcome to icliniq.com.

I understand your concern.

Folfox is a name given to drugs, which include injection of Oxaliplatin, injection of Leucovorin, and injection of Fluorouracil. These are chemotherapy drugs. These dosages are decided on the basis of body surface area, so if, as per her body surface area, she should get 100 milligrams of injection of Oxaliplatin, then she will give 50%, which means 50 milligrams.

The same goes for the injection of Leucovorin and injection Fluorouracil. The standard dose for Oxaliplatin is 85 mg/m², injection of Leucovorin 400 mg/m², and injection of Fluorouracil is 2400 mg/m². So we will calculate her body surface area, and then she should get a 50% dose in the first cycle. And if tolerated well, then 75% of the dose in the remaining cycles.

I hope this helps.

Kindly revert if there are any queries.
