Hello doctor,

My mom, age 63, with stage 3 breast cancer, developed severe confusion and slurred speech during radiation. A CT (computed tomography) head shows three small lesions. We are waiting for her MRI (magnetic resonance imaging). The steroid dose doubled, but she is becoming more agitated.

Blood sugar is greater than 400 despite insulin. Left arm suddenly swollen and painful. Her ultrasound is scheduled for next week. The oncology team suggests whole brain radiation (WBR) but is concerned about side effects.

How effective is WBR? Are there newer treatments for brain metastases? Given her condition, what is her prognosis? Struggling with whether to continue aggressive treatment or focus on comfort care.
