Hi doctor,

I have four cracked teeth and one that might be starting to crack. I looked it up, and it seems like they are craze lines or oblique supragingival cracks because I do not have any symptoms. Most of the cracks are not very noticeable and are only on the part of the tooth that is closer to where I chew. There is one tooth where I have a very thin line going all the way up to the gums, but I can see it only when my teeth are not brushed for a while. But that is one of my front teeth, so I do not want it to get worse. I have another tooth in the top row that has several ripples on it, but I do not know if it is cracked yet. The most noticeable crack is on a tooth in the bottom row, and I have a few other cracked teeth in the bottom row, as well. I was just wondering if I need to see a dentist urgently about this or if I can wait a few weeks.

Also, I went to the dentist two months back to get a wisdom tooth pulled because I was having pain in it when drinking or eating sweets, and they said it was impacted. But after I got it pulled, I continued to have pain (under the same circumstances). It also spread to more and more teeth over time and has become increasingly painful. But I was having pain less and less often, as well. It has been several months now since the last time those teeth hurt. But I thought I would also bring it up because I similarly wanted to know if it was urgent. I have attached my X-rays. Please help.

I think your response got cut off at the end. But are you sure it is on the upper right side? The pain is actually in the upper left side. And no, I have not been to the dentist since the extraction. My main question was about the cracks in my other teeth, though. Please advise.

I understand your concern.

I was looking at the mirror image of the X-rays (attachments removed to protect patient privacy). The tooth cracks that you are noticing are not as prominent on the X-ray as in the upper left area. But even after your wisdom tooth has been extracted, if your other premolars and molars, or the posterior teeth in the upper left are paining, it is definitely more due to a gum infection, rather than the tooth itself. I can observe a bit of bone loss as well in this segment. You will need to get deep dental scaling or cleaning done. Also, I would suggest that you start by applying Dentogel (Choline salicylate 8.7 percent w/w, Lidocaine 2 percent w/w, and Benzalkonium chloride 0.01 percent w/w) regularly every night. I also suggest brushing your teeth with a different remineralization paste at night. You can use your regular paste in the morning. Night be particular in using this anti-cavity and remineralizing toothpaste. Do this regularly from now on. These are only preliminary measures that you can take to prevent your tooth pain and enamel from further deteriorating.

Now that you mention this pain has been persisting for several months, did you get a CBCT (Cone beam computed tomography) done? What happens in a CBCT is that if any tooth line is fractured amongst the individual teeth in that upper left segment, it will be detected more clearly and also we will get to know if any cysts, any fractures, or any bone is chipped off around those teeth that is causing you so much pain.
